Current condition of facility in photos enclosed(EXHIBIT L). • Item#403 - CCR 66265.173 - Failed to keep hazardous waste containers closed except when adding or removing hazardous waste. o CORRECTIVE ACTION: ■ Hazardous Waste - Container Management • As is Dollar Tree's standard practice, all of the hazardous waste collected (above) was processed,placed in proper containers(55-gallon drums),and stored for pickup by our vendor Stericycle. This included the potential hazardous waste found draining into a small container at the Hazardous Waste Accumulation Area (EXHIBIT M). • Item#404 - CCR 66265.174 - Failed to inspect hazardous waste storage areas at least weekly. o CORRECTIVE ACTION: ■ Hazardous Waste—Weekly Inspections • Weekly inspections are only conducted onsite by a member of the Maintenance Management group. (Facilities Maintenance Manager, L2 Maintenance Manager & L3 Maintenance Manager) (See EXHIBIT N for representative inspection forms). • These Managers rotate inspections on a weekly basis to ensure compliance across all shifts. • Additional program validation has been developed & incorporated into the Hazardous Waste Program which shall include monthly, quarterly & bi-annual inspections. • Item #407 - CCR 66265.17(b), 66265.177 - Failed to manage incompatible wastes in containers properly. o CORRECTIVE ACTION: ■ Hazardous Waste Management- Incompatible Waste • It appears an Associate may have misidentified the containers for waste and product management. The products found in the small tote were subsequently scanned, processed and placed in the appropriate drum (which is the approved 55-gallon, closed-top drum). • The cloth was disposed of as hazardous waste. The Associate who may have been responsible for these specific materials was retrained on Oct 16, 2019. This area continues to be well-managed. • Item #605 - CCR 66262.34(f) - Failed to completely label containers or portable tanks of hazardous waste. o CORRECTIVE ACTION:
