The key actions taken included identifying a central repack area, adding 2 more <br /> technicians to the Maintenance/Hazardous Waste function, adding 6 more <br /> Housekeepers, and providing additional training on the "Clean as You Go" <br /> standards. Current condition of facility in photos enclosed(EXHIBIT L). <br /> • Item#403 - CCR 66265.173 - Failed to keep hazardous waste containers closed except when <br /> adding or removing hazardous waste. <br /> o CORRECTIVE ACTION: <br /> ■ Hazardous Waste - Container Management <br /> • As is Dollar Tree's standard practice, all of the hazardous waste collected (above) <br /> was processed,placed in proper containers(55-gallon drums),and stored for pickup <br /> by our vendor Stericycle. This included the potential hazardous waste found <br /> draining into a small container at the Hazardous Waste Accumulation Area <br /> (EXHIBIT M). <br /> • Item#404 - CCR 66265.174 - Failed to inspect hazardous waste storage areas at least weekly. <br /> o CORRECTIVE ACTION: <br /> ■ Hazardous Waste—Weekly Inspections <br /> • Weekly inspections are only conducted onsite by a member of the Maintenance <br /> Management group. (Facilities Maintenance Manager, L2 Maintenance Manager <br /> & L3 Maintenance Manager) (See EXHIBIT N for representative inspection <br /> forms). <br /> • These Managers rotate inspections on a weekly basis to ensure compliance across <br /> all shifts. <br /> • Additional program validation has been developed & incorporated into the <br /> Hazardous Waste Program which shall include monthly, quarterly & bi-annual <br /> inspections. <br /> • Item #407 - CCR 66265.17(b), 66265.177 - Failed to manage incompatible wastes in <br /> containers properly. <br /> o CORRECTIVE ACTION: <br /> ■ Hazardous Waste Management- Incompatible Waste <br /> • It appears an Associate may have misidentified the containers for waste and product <br /> management. The products found in the small tote were subsequently scanned, <br /> processed and placed in the appropriate drum (which is the approved 55-gallon, <br /> closed-top drum). <br /> • The cloth was disposed of as hazardous waste. The Associate who may have been <br /> responsible for these specific materials was retrained on Oct 16, 2019. This area <br /> continues to be well-managed. <br /> • Item #605 - CCR 66262.34(f) - Failed to completely label containers or portable tanks of <br /> hazardous waste. <br /> o CORRECTIVE ACTION: <br />
