The incorrect and missing information included the following from the inspection reports: <br /> -The DO report of 11/23/20 Section 5 was left blank. The owner/operator failed to provide explanation of follow-up <br /> actions that will be taken for the statement provided by the DO in section 3. <br /> REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> This is a repeat violation, Class Il. <br /> 306 HSC 25291(e) Failure to remove, analyze, and properly dispose of water in secondary containment(pre-Jul 2003). <br /> OBSERVATION: Liquid was observed in the 87, diesel fill sumps, and 87 spill bucket. <br /> REGULATION GUIDANCE: If water could enter into the secondary containment by precipitation or infiltration, it <br /> must be removed and disposed of properly. <br /> CORRECTIVE ACTION: The service technician removed approximately three gallons of liquid from the 87, 91 fill <br /> sump and 87 spill bucket. Ensure that all sumps and annular spaces are maintained free of liquid. <br /> This was corrected on site. <br /> This is a minor violation. <br /> 319 CCR 2635, 2637.2, 2665, 2712 Failure to comply with overfill prevention equipment requirements. <br /> OBSERVATION: Overfill prevention testing has not been conducted. <br /> REGULATION GUIDANCE: Overfill prevention equipment inspections were required to be conducted by October <br /> 13, 2018, and every 36 months thereafter, and within 30 days of completion of a repair. <br /> CORRECTIVE ACTION: Immediately schedule this testing and provide 48 hours notification to the EHD.
