The following is an itemized list of underground storage tank violations that have not <br />been addressed for GRANT LINE SHELL* as of July 22, 2021. <br />Open violations from May 26, 2021 inspection <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Violation #208 - Failure to comply with the designated operator inspection requirements. <br />OBSERVATION: Designated operator reports for the past thirty-six months were not found on site. <br />REGULATION GUIDANCE: Designated operator inspection reports prior to October 1, 2018 shall be retained on <br />site for twelve months. Designated operator inspection reports on or after October 1, 2018 shall be retained on site <br />for thirty-six months. <br />CORRECTIVE ACTION: Ensure designated operator reports are being conducted every 30 days and are kept on <br />site for thirty-six months. <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Violation #209 - Failure to maintain a copy of the "Facility Employee Training Certificate" on-site. <br />OBSERVATION: No Facility Employee Training Certificates were observed on site. <br />REGULATION GUIDANCE: At least one of the facility employees present during operating hours shall have current <br />training. A list of facility employees who have been trained by the designated UST operator(s), <br />shall be maintained on-site or off-site at a readily available location, if approved by the <br />local agency. All site employees must be trained prior to taking responsibility as a facility employee if after October <br />1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain <br />accurate information for all employees trained. <br />CORRECTIVE ACTION: Ensure there is a currently trained facility employee on site during operating hours and <br />training certificates are kept on site. <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Page 3 of 6 <br />
