The following is an itemized list of underground storage tank violations that have not <br />been addressed for GRANT LINE SHELL* as of July 22, 2021. <br />Open violations from May 26, 2021 inspection <br />Violation #210 - Failure to provide initial training, train every 12 months, and have a trained employee <br />present. <br />OBSERVATION: No trained employees were on site. The designated operator employee training for Anthony Brown <br />and Donte Mcclain Mitchell were not performed. Both employees have stated that they have not had DO training or <br />seen DOs during their work shifts. Anthony stated he started 1.5 weeks ago and Donte stated he started working 2 <br />weeks ago. Both employees were the only employees. <br />REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and <br />maintenance of the UST system at least once every 12 months. The training shall be conducted before the <br />individual performs the duties of a facility employees. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Initial training shall also include a site-specific practical demonstration that provides both sharing information and <br />evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least <br />one of the facility employees present during operating hours shall have current training. <br />CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on <br />site, and submit a copy of the training records to the EHD. <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Violation #214 - Failure to comply with UST operating permit conditions. <br />OBSERVATION: The leak detectors on the tank closest to the store were removed and the 89 grade tank (end tank <br />closest to the store) was converted to 87 and a permit was not obtained from the EHD. <br />REGULATION GUIDANCE: Emergency repairs relating to UST system monitoring and release detection may be <br />performed by a properly licensed, trained, and certified contractor pursuant to the EHD's policy and a permit <br />obtained within one business day from the EHD. All other repairs must have a permit submitted to and approved by <br />the EHD prior to beginning work. <br />CORRECTIVE ACTION: Immediately obtain a permit from the EHD for the tank conversion.
