The following is an itemized list of underground storage tank violations that have not been addressed for GRANT LINE SHELL* as of July 22, 2021. Open violations from May 26, 2021 inspection Violation #210 - Failure to provide initial training, train every 12 months, and have a trained employee present. OBSERVATION: No trained employees were on site. The designated operator employee training for Anthony Brown and Donte Mcclain Mitchell were not performed. Both employees have stated that they have not had DO training or seen DOs during their work shifts. Anthony stated he started 1.5 weeks ago and Donte stated he started working 2 weeks ago. Both employees were the only employees. REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual performs the duties of a facility employees. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Initial training shall also include a site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least one of the facility employees present during operating hours shall have current training. CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #214 - Failure to comply with UST operating permit conditions. OBSERVATION: The leak detectors on the tank closest to the store were removed and the 89 grade tank (end tank closest to the store) was converted to 87 and a permit was not obtained from the EHD. REGULATION GUIDANCE: Emergency repairs relating to UST system monitoring and release detection may be performed by a properly licensed, trained, and certified contractor pursuant to the EHD's policy and a permit obtained within one business day from the EHD. All other repairs must have a permit submitted to and approved by the EHD prior to beginning work. CORRECTIVE ACTION: Immediately obtain a permit from the EHD for the tank conversion.
