The following is an itemized list of underground storage tank violations that have not <br />been addressed for GRANT LINE SHELL* as of July 22, 2021. <br />Open violations from May 26, 2021 inspection <br />Violation #301 - Failure to operate UST system to prevent unauthorized releases including leaks, spills and <br />overfills. <br />OBSERVATION: Product piping was found not contained within the UDC footprint for the following dispensers: 1/2, <br />5/6, 11/12, 15/16, 17/18, 19/20, 21/22. If a leak were to occur from the product piping, the leak would spill onto the <br />ground and not into the UDC containment. <br />REGULATION GUIDANCE: The underground storage tank system shall be operated to prevent unauthorized <br />releases, including spills and overfills. <br />CORRECTIVE ACTION: Immediately make corrections to contain piping within the UDC should a leak occur under <br />permit and inspection by EHD, if necessary. <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Violation #319 - Failure to comply with overfill prevention equipment requirements. <br />OBSERVATION: The 87, 89, 91, Diesel does not have an overfill prevention system. The tank form lists fill tube <br />shut-off valve as the sole overfill prevention method, but flappers were not observed on the drop tubes during the <br />inspection. Prior CERS submittals have reported ball floats as the site's overfill method. <br />-On 5/12/2020 EHD received an Overfill Prevention Inspection Report dated 5/2/2019. A notification was previously <br />submitted to EHD. 48-hour notifications are required for testing being conducted on UST systems. A test report was <br />not observed during the annual inspection. EHD will not accept this testing. <br />- A permit was approved and emailed to remove ball floats and install overfill prevention equipment in the 87, 89, 91, <br />and diesel tanks was emailed 2/16/2021 but work on this permit has not yet been conducted. <br />REGULATION GUIDANCE: All underground storage tanks shall be equipped with an overfill prevention system that <br />does not allow for manual override and meets one of the following requirements: <br />1. Alert the transfer operator when the tank is 90 percent full by restricting the flow into the tank or triggering an <br />audible and visual alarm; or <br />2. Restrict delivery of flow to the tank at least 30 minutes before the tank overfills, provided the restriction occurs <br />when the tank is filled to no more than 95 percent of capacity; and activate an audible alarm at least five minutes <br />before the tank overfills; or <br />3. Provide positive shut-off of flow to the tank when the tank is filled to no more than 95 percent of capacity; or, <br />4. Provide positive shut-off of flow to the tank so that none of the fittings located on the top of the tank are <br />exposed to product due to overfilling. <br />CORRECTIVE ACTION: Immediately contact a properly licensed, trained, and certified contractor to install an <br />approved overfill prevention system. Replacement of a mechanical flapper valve requires a permit and inspection <br />by the EHD. If an external audio/visual alarm is replaced, provide a copy of the service receipt to the EHD. If an <br />approved overfill prevention system can't be installed immediately, there is a possibility that the 87, 89, 91, Diesel <br />UST system may be red tagged to prevent fuel inputs. <br />Page 5 of 6 <br />
