Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: CO0052303 Site Location: 435 W CALLADO AVE <br /> Activity Summary <br /> Activity Date Recorded by <br /> 08/12/2020 MAILED REGULAR NOTICE TO ABATE & PHOTO LOG DATED 08/07/2020 TO BALLWAHN <br /> PROPERTY OWNER. <br /> VIKRANT JAIN <br /> PO BOX 513 <br /> NEW YORK NY 10024-0513 <br /> 08/13/2020 Property manager Sujjan (408) 940-5021 LINDSAY <br /> called and let me know he received my letter in the mail. He wanted to explain his <br /> side of things. He said the tenant moved in May on a month to month lease. They <br /> clogged the toilet and they had a plumber come out and fixed everything back up. <br /> Then two months later they clogged the toilet again and there was an excessive <br /> amount of water, the plumber came out and fixed it. Now the tenants daughter is <br /> complaining about mold and they just redid the flooring in May. <br /> I told him that I went out for an investigation and saw the floor boards slightly lifted, <br /> the baseboards had water stains and the carpet in the bedroom had a slight odor <br /> to it. I recommended changing the carpet and the owner said he would redo the <br /> baseboards too. I said that would be enough to close the complaint. He said the <br /> tenant is suppose to move out end of the month as they are not renewing the <br /> lease for another month. I let him know I can give him till early October to <br /> complete the work. I asked him to stay in contact with me about progress on the <br /> house. <br /> 10/12/2020 Called property management-to discuss the extension of compliance schedule LINDSAY <br /> and any questions he had. He asked if he had to have the house tested for mold, I <br /> told him if he is willing to replace the carpet and ruined base and floor boards then <br /> I am not required him to test for mold. He said he would replace the carpet, check <br /> the floor boards, and repaint the baseboards. <br /> Told him we would extend the compliance schedule till end of November. <br /> 12/21/2020 Received photos from owner showing the violations cited had been corrected. LINDSAY <br /> Also received a voicemail from the owner during this time. Emailed owner back <br /> and let him know I would be closing the complaint and he would receive <br /> paperwork in the mail. <br /> Filled out complaint form and printed 5105. <br /> Filled out OIR stating that the complaint is closed. Printed photos from owner <br /> showing violations are corrected. <br /> Gave to Scott to review. <br /> 5105.rpt Page 2 of 2 <br />
