CAUTION:This email is originated from outside of the organization. Do not click links or open attachments unless you recognize the
sender and know the content is safe.
Hi Robert,
I understand that you have been in communication with Paloma Cabrera, Regional Manager, regarding our Stockton
Diagnostic Imaging-California Street location. I have reviewed the Hazardous Materials Program Inspection Report in detail
and have consulted with our Physicist, Dr. Heindrick Vartani. He has confirmed that we do not store hazardous materials
that are at or above the thresholds that are outlined in the report; quantities of 55 gallons for materials that are liquids, 500
pounds for solids, or 200 cubic feet for compressed gas. I have also consulted with our Quality Assurance Department and
validated this as well.
I would like the opportunity to discuss as I don't believe that we fall within the regulatory requirement that you issued a
corrective action Class II violation for. I left you a detailed voice message at 3:27pm this afternoon and requested a
returned phone call. You may reach me at(916) 752-6451.
Warm Regards,
Melanie
Ok Melanie Santiago I Vice President of Operations I Northern California I Mobile: 916-752-6451
Melan ie.Santiago c( rad net.corn I radnet.com
From: Robert Lopez [EHD] [mai Ito:rlopez sjgov.org]
Sent: Thursday, February 25, 2021 3:47 PM
To: Paloma Cabrera
Subject: PR0521912; Stockton Diagnostic Imaging Hazardous Materials Program Inspection
WARNING: Outside Sender
Mr. Cabrera,
I have attached a copy of the Hazardous Materials Program Inspection Report, Inspection Photos, Corrective Action
Statement Form, Map Guidance Documents, Emergency Response/Contingency Plan template, and a sample employee
training record.
As of September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
