Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: CO0046889 Site Location: HWY 99 FRONTAGE RD <br /> Activity Summary <br /> Activity Date Recorded by <br /> 06/08/2018 Was informed of incident at 8:00 by oncall person. Was told to travel to site and RUVALCABA <br /> meet oncall person to discuss the situation with the cleanup that was happening. <br /> Traveled to site shortly after 8:00 am and met with inspector Elena Manzo and <br /> Parc Environmental. Elena had responded to this call after hours. Walked Kingsley <br /> Rd approximately .5 mile south, starting at the Sunny Rd. intersection. Several <br /> light blue round pill were observed. Pills that were not broken could had an M on <br /> one side and a 30 on the other side. Parc personnel identified the pills as <br /> Oxycodone HCI Tablets, 30 miiligrams. This was done by typing the description of <br /> the pill into an internet search engine. A picture pulled up the very much <br /> resembled the pills in question. Elena received a phone call from the CHP and <br /> was informed that the laboratory analysis showed the pills contained Fentanyl <br /> although concentration was not given. Light blue fragments of what appeared to <br /> be from the pills were observed along the walk. The pills and fragments of pills <br /> were dispersed along the mile. Parc Environmental used a dust pan and a small <br /> broom/brush to sweep up the pills. The recovered pills and similarly colored <br /> fragments recovered were placed in a 5 gallon plastic container. After speaking <br /> with the Program Coordinator and with Michael Pixton with DTSC (510) 589-2981 <br /> and discussing the disposal of the pills and fragments, Michael stated that Parc <br /> Environmental was doing the correct thing. Parc Environmental wanted someone <br /> from the county to sign the hazardous waste manifest. After consulting with <br /> Program Coordinator, a member of the County Hazardous Materials Team was <br /> dispatched to sign the manifest. Left site at 12:15 pm. Park Environmental <br /> remained onsite to wait for signature. <br /> Logged photos. <br /> 06/08/2018 Onsite with Cesar Ruvalcaba, met with Parc Environmental clean up clrew, MANZO <br /> discussed the clean up strategy, they have a Hepa filter vacuum they will use to <br /> recover fragments and pick up all the pills that are not damaged. Walked the <br /> impacted area with Cesar to delineate the extent of the impact. Called Scott <br /> Wight(DEA)while onsite 916-217-7789, he indicated that they got results back <br /> from the laboratory and the substance is fentanyl laced with tylenol. Discussed site <br /> safety with Cesar and to avoid exposure during clean up. <br /> 06/08/2018 7 am -7:12 am, call to Roger with Parc Environmental confirmed the ETA and MANZO <br /> clean up activities; they will be onsite a little before 9 am. <br /> 06/08/2018 At the request of Muni, I responded to Sunny Rd. at State Route E 99 Frontage LOPEZ <br /> Rd. to sign waste removal manifest and work log. <br /> 06/11/2018 Completed 004 daily narratives for 6/8/18; forwarded waste manifest and work log LOPEZ <br /> to Cesar Ruvalcaba. <br /> 06/14/2018 Complaint organization and narrative. MANZO <br /> 08/08/2018 Emailed accounting update on responsible party, who is currently incarcerated. RUVALCABA <br /> Public defender has asked invoice to be sent to them. <br /> 08/15/2018 Returned call to public defender. Forwarded email to accounting. There were RUVALCABA <br /> questions on payments since the responsible party is incarcerated. <br /> 01/14/2021 Received complaint package from C. Ruvalcaba (assigned EHD inspector), MANZO <br /> complaint file was lacking the final disposal record, picture and ER record. Located <br /> and printed TSDF manifest copy, pictures, map, and ER record. <br /> Abated complaint. <br /> 5105.rpt Page 3 of 3 <br />
