Complaint Investigation Summary Report#:5105 COMPLAINT ID: CO0052167 Site Location: EAST BOUND HWY 26 EAST OF FINE RD Activity Summary Activity Date Recorded by 10/07/2020 After discussions with Elena Manzo, Ld Sr RENS, Brandon Espinoza of All FLORIDO Environmental, and Juan Armendarez of Cal Trans, it was determined that according to Juan and based on a comparison of the photos from CHP and the photo of the final clean-up, the 8-10 inches of soil that had been placed over the gasoline spill by the construction company, had been removed prior to All Environmental conducting the gasoline spill clean-up, thus the gasoline spill has been adequately excavated. Thus, the clean-up is complete. During my discussion with Juan I asked him what waste was left on-site on the date of the incident, since Claudia had previously told me that she was told waste had been left on-site. Juan stated the absorbent that had been applied to the diesel spill on the roadway had been pushed off to the shoulder of the road, which was excavated yesterday, so the used absorbent was also picked up during the excavation. Discussed Juan's statement regarding the absorbent with Claudia, who agrees that the absorbent was the only waste she was aware of that had been left on-site. 10/07/2020 Printed and reviewed waste soil disposal record received on 10/6/2020 from FLORIDO Brandon Espinoza. 10 cubic yards of excavated soil was disposed of using a non-hazardous special waste manifest at Forward Landfill in Manteca. 10/08/2020 Reviewed soil sample analytical collected for waste disposal purposes received on FLORIDO 10/7/2020 from Brandon Espinoza of All Environmental. The analytical is as follows: BTEX: Benzene- non detect(ND) Toluene-0.34 mg/kg Ethylbenzene-2.1 mg/kg Total Xylenes- 1.7 mg/kg Total Petroleum Hydrocarbons (TPH): Diesel Range- 14000 mg/kg pH: 7.19 11/19/2020 Reviewed and revised narrative. Printed and organized complaint documentation FLORIDO according to new complaint abatement procedures. 01/07/2021 - Reviewed narrative since complaint is going to be abated. Made revisions to MURO areas requiring additional information or clarification. - Converted emails related to the complaint into pdfs. - Emailed traffic collision report and emails to Elianna Florido. 01/11/2021 Reviewed and revised narrative.
