Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: CO0052167 Site Location: EAST BOUND HWY 26 EAST OF FINE RD <br /> Activity Summary <br /> Activity Date Recorded by <br /> 08/20/2020 Emailed Brandon and Juan the following: "Any update on getting the gasoline spill FLORIDO <br /> caution taped off so the construction equipment doesn't keep running over it? <br /> Also, any update on possibly getting this clean-up completed on a day when there <br /> is no construction activity? <br /> 09/17/2020 Received an email with invoice from EHD attached from Juan Armendarez, MURO <br /> juan.armendarez@dot.ca.gov, on 09-16-2020. Juan is inquiring about an invoice <br /> he received and is hoping is an error. <br /> I responded to Juan Armendarez letting him know I will look into it. <br /> Discussed issue with Elena Manzo to let her know I had updated the responsible <br /> party on complaint investigation form to reflect that which was disclosed by <br /> Sergeant Barbara Butler(CHP) via 07-07-2020 email. She advised I discuss with <br /> accounting. <br /> Discussed issue with Michelle Ozuna, from EHD Accounting Department. Michelle <br /> Ozuna confirmed that the invoices were going to Caltrans. Michelle updated <br /> responsible party information and asked that I tell Caltrans that they may still <br /> receive another invoice, but to disregard. <br /> Emailed Juan Armendarez to let him know issue was resolved. <br /> 09/29/2020 Received an email from Juan Armendarez notifying me that All Environmental will FLORIDO <br /> be cleaning the spill on 10/6/20 at 9am. Emailed Juan and Brandon that EHD will <br /> verify the clean-up is complete, and asked Brandon if he could bring a PID <br /> detector on the date of the clean-up. <br /> 09/29/2020 Reviewed narrative. FLORIDO <br /> 10/06/2020 Received a call from Brandon Espinoza of All Environmental, stating the clean-up FLORIDO <br /> will be done around 9am this morning. <br /> Arrived on-site and met with Brandon Espinoza and Juan Armendarez of Cal <br /> Trans. Verified the diesel and gasoline clean-ups were complete. The excavated <br /> soil was placed in a roll-off bin. Took photos of the excavated areas and <br /> excavated waste soil. Brandon stated the soil will be disposed of at Forward <br /> Landfill in Manteca, and that he will provide me with the waste sample analytical, <br /> waste profile, and disposal record. Left site for home. <br /> Downloaded inspection photos and generated photo log. <br /> 10/06/2020 Reviewed narrative and complaint photos. Determined the clean-up of the FLORIDO <br /> gasoline spill may not be complete, since All Environmental may not have dug <br /> deep enough. <br /> 5105.rpt Page 7 of 8 <br />
