Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: C00052583 Site Location: 192 E LATHROP RD <br /> Activity Summary <br /> Activity Date Recorded by <br /> 10/08/2020 Called the Penske Facility at 3663 Petersen Rd in Stockton. Spoke to the Philip FLORIDO <br /> Gomes, Service Manager, who requested I send him photos of the incident, truck, <br /> and reefer trailer, so that he may determine if the reefer trailer in fact was towed to <br /> his facility. Sent Philip the photos that were provided to me by Patriot <br /> Environmental, via text message. Emailed Brian Yazemboski requesting the VIN <br /> number for the reefer trailer or the license plate number, and the name of the <br /> towing company. <br /> Received a call from Philip who indicated that the trailer was not towed to his <br /> facility, and he referred me to Trevor Ingram, Operations Supervisor at Penske's <br /> Dispatch for Trader Joe's. Called and spoke to Trevor and provided him the <br /> information. Trevor could not find the name of the towing company nor where the <br /> trailer had been towed to. Trevor provided me with PLM's (PLM rented the trailer <br /> to Penske for Trader Joe's, according to Philip Gomes) roadside assistance phone <br /> number and OM Alfaro's Trucking (carrier/driver) phone number. <br /> Called PLM roadside assistance and spoke to PLM's dispatch for existing vehicle <br /> breakdowns. The PLM dispatcher was unable to find the information regarding the <br /> affected trailer, since I don't have a reference number. <br /> Called and left the driver of the trailer a voice message to call me back. <br /> Discussed complaint with Garrett Backus, Ld Sr REHS. <br /> Received a call back from the trailer driver, who states he took the trailer to VTR <br /> Stockton, 3798 S Highway 99 in Stockton, to be repaired. He states he doesn't <br /> know what VTR did with the impacted diesel tank containing the absorbent. <br /> Called VTR (Valley Transport Refrigeration) Stockton and spoke to Janette, office <br /> assistant. Janette stated she would call me back if she found the Penske trailer <br /> with the affected diesel tank in her system. Received a call back from Janette who <br /> confirmed her facility repaired a diesel tank on a Penske trailer on 9/9/2020. <br /> Janette transferred me to RJ, mechanic. Spoke to RJ who stated the affected <br /> diesel tank was replaced and removed from the trailer and placed on a pallet at <br /> VTR. RJ indicated that the absorbent inside the tank was not removed from the <br /> tank. RJ stated the diesel tank would be picked up by VTR's hazardous waste <br /> company, if it hadn't been already. RJ could not confirm at this time if the diesel <br /> tank with the absorbent had already been picked up for disposal by the hazardous <br /> waste hauler. <br /> 10/08/2020 Reviewing narrative and photos provided to me by Patriot Environmental. FLORIDO <br /> 11/17/2020 Reviewed and revised narrative, per new complaint abatement procedures. FLORIDO <br /> Reviewed and gathered complaint documentation. Complaint to be abated to the <br /> Valley Transport Refrigeration facility file (PR0522169), for their disposal of the <br /> absorbent used to absorb the 1-2 gallons of diesel remaining in the affected diesel <br /> tank. <br /> 11/18/2020 Printed and organized complaint documentation according to new complaint FLORIDO <br /> abatement procedures. Submitted complaint documentation to Elena Manzo, Ld <br /> Sr REHS. <br /> Refer to Valley Transport Refrigeration hazardous waste file (PR0522169) for <br /> disposal records for the absorbent used to absorb the 1-2 gallons of diesel that <br /> remained inside the affected diesel tank. <br /> 5105.rpt Page 3 of 3 <br />
