Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: CO0052713 Site Location: 1630 E HAMMER LN <br /> Activity Summary <br /> Activity Date Recorded by <br /> 09/28/2020 Generated Prop 65, Emergency Response Record, and photo log. FLORIDO <br /> 10/08/2020 Reviewed and revised narrative, and printed and organized complaint FLORIDO <br /> documentation per EHD's new complaint abatement procedures. <br /> 11/16/2020 Reviewed a letter and waste manifest received on 11/13/2020 from Charle Morris, FLORIDO <br /> Costco manager. The waste TSDF manifest is for one drum (100 pounds) of <br /> waste solids containing flammable liquid, which includes the waste generated from <br /> the spill clean-up. The letter confirms there are no other concerns that need to be <br /> followed up on, and the facility has reviewed proper spill cleanup procedures with <br /> all attendants. <br /> 11/16/2020 Reviewed and revised narrative. Verified complaint documentation is organized FLORIDO <br /> according to new complaint abatement proceudres. Submitted complaint <br /> documentation to Elena Manzo, Ld Sr REHS, for abatement. <br /> 12/18/2020 Received a call from Charle Morris, Costco Asst. General Manager, asking if any FLORIDO <br /> further information was needed. Reviewed complaint narrative and details. <br /> Emailed Charle letting her know I received a hard copy of her letter and the waste <br /> manifest last month, my supervisor is reviewing the complaint file, and if any <br /> further information is needed I will let her know. <br /> 01/06/2021 Reviewed and completed complaint documentation revisions from Elena Manzo, FLORIDO <br /> Ld Sr REHS.
