Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: CODMUHFOO Site Location: 833 East 8th St <br /> Activity Summary <br /> Activity Date Recorded by <br /> 08/21/2020 5-7 pm. Called Derek Smith, Patriot Environmental and he told me the spill BACKUS <br /> cleanup is nearing completion. I told him I'll be coming by within 20 minutes. <br /> Once on-site, I spoke with Derek Smith and Paul Masson, Union Pacific <br /> consultant. An Union Pacific representative was present but did not identify <br /> himself. Paul took my card and said he will email an incident report. <br /> According to Paul, the spill occurred from the west side of the locomotive only and <br /> spilled diesel onto soil in between two sets of tracks. The spill report lists 100 <br /> gallons spilled. Contaminated soil was excavated and placed in a Patriot roll-off <br /> bin. The cleanup appeared to excavate the contaminated soil from the current spill <br /> but apparent contamination from previous incidents was observed, especially on <br /> the west side of the excavation next to adjacent railroad tracks . Paul stated that <br /> the contamination on the west side of the excavation did not smell like diesel . He <br /> thought it was a heavier oil from a previous incident. Patriot Environmental used a <br /> portable meter to detect the diesel spill and determine where to dig. <br /> Derek Smith said the roll-off bin of contaminated soil will be left on-site until <br /> profiling. I told him that I'll need to get the disposal record and profile to close the <br /> complaint. <br /> 08/24/2020 Complaint was taken over by lead senior Garrett Backus. Printed out the ALANIZ <br /> complaint information and passed it to Garrett. <br /> 10/02/2020 Emailed Derek Smith (Patriot Env) and Mike Algots (UPRR) to get the disposal BACKUS <br /> record for the diesel contaminated soil and the waste profile. <br /> Derek emailed me back stating that"ghd" has not provided the information to him <br /> yet. He'll provide it when available. <br /> Mike Algots emailed me and said he will provide the records when available. <br /> UPRR reference number is WR 2080. <br /> 12/14/2020 Emailed Mike Algots for the waste profile and disposal record. BACKUS <br /> 12/17/2020 Received Email from Mike Algots. GHD will be providing the disposal record and BACKUS <br /> waste profile. <br /> Printed ER report and created photo log. <br /> 01/04/2021 Reviewed soil analytical results for samples taken 10-13-20. Data was received BACKUS <br /> 12-18-20. Metals, Total Petroleum Hydrocarbons and Volatile Organic <br /> Compounds were analyzed and TTLC and TCLP amounts for metals were below <br /> Title 22 CCR 66261.24 hazardous waste levels. The sample did exceed STLC <br /> hazardous waste levels for lead (11.2 ppm). The regulatory limit is 5 ppm. <br /> I emailed Mike Algots to get the disposal record. <br /> 01/05/2021 Received and filed UHWM 019703805 JJK, dated 11-13-20, for 18 cu yds of lead BACKUS <br /> contaminated soil.
