Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: CODYOYNN6 Site Location: 21329 E MARIPOSA RD <br /> Activity Summary <br /> Activity Date Recorded by <br /> 10/02/2020 Received phone call from John Weeks (property owner of 21329 Mariposa). He BAKER <br /> said there were two drums on site since 9/2/2020 and wanted to know when they <br /> would be picked up. I told him I would find out or pass it on the Elianna Florido <br /> (Sr. REHS). Looked at complaint narrative. Called Brandon Espinoza of All <br /> Environmental and he said he would go and pick them up Monday. Called John <br /> Weeks back and left a message letting him know. <br /> 10/07/2020 Received a call from John Weeks, owner of property where spill occurred, who FLORIDO <br /> states he spoke to someone from EHD who told him that the two drums of diesel <br /> were going to be picked up on 10/4/20, but are still on his property. Lydia Baker, <br /> REHS, told me that she spoke to Brandon Espinoza of All Environmental who told <br /> her that he would pick up the diesel drums on 10/4/20. Called Brandon, but was <br /> unable to leave him a voice message due to his message box being full. Sent <br /> Brandon a text message asking him to give me an update on the diesel drums. <br /> Received a call back from Brandon who states he will pick up the drums tomorrow <br /> and will be giving them to the responsible party, Valley Farm Transport, who will <br /> use them as product. Called John Weeks to let him know that Brandon will be <br /> giving John a call tomorrow when he is on his way to pick up the drums. <br /> 10/08/2020 Received a call from Brandon Espinoza and reminded him that I still need the FLORIDO <br /> waste soil disposal records for this spill clean-up. Brandon also stated either Valley <br /> Farm Transport or Brandon himself will use the two drums of diesel as product. <br /> 11/12/2020 Received a call from Alex Lakeman, Valley Farm Transport Compliance FLORIDO <br /> Administration and answered her questions regarding the billing for this complaint. <br /> 11/19/2020 Emailed Brandon Espinoza of All Environmental requesting the disposal records FLORIDO <br /> for the waste soil and two drums of diesel generated from this spill clean-up. <br /> Called and spoke to Matthew Nickum of Valley Farm Transport, to request the <br /> disposal records; Matthew referred me to Alex Lakeman, Valley Farm Transport <br /> Compliance Administrator. Emailed Alex Lakeman, requesting the disposal <br /> records. Received a call from Alex who stated she does not have the disposal <br /> records and that the clean-up contractor should have them. I explained to Alex <br /> that Valley Farm Transport will be billed for my time spent requesting the disposal <br /> records. Alex emailed Brandon requesting he provide me with the disposal <br /> records. <br /> 11/19/2020 Reviewed and revised narrative. Printed and organized complaint documentation FLORIDO <br /> according to new complaint abatement procedures. <br /> 11/30/2020 Reviewed disposal record for roll-off bin of soil contaminated with oil/diesel, FLORIDO <br /> received on 11/25/20 from Brandon Espinoza of All Environmental. Emailed <br /> Brandon and asked him what happened to the two drums of diesel, which was <br /> removed from the truck involved in the accident. Brandon emailed stating the two <br /> drums of diesel were used as product. <br /> 11/30/2020 Revised narrative. Printed narrative and submitted complaint documentation to FLORIDO <br /> Elena Manzo, Ld Sr REHS, for abatement. <br /> 12/14/2020 Reviewed narrative. Emailed Alma Gonzales, David Morse Senior Adjuster, a copy FLORIDO <br /> of my complaint narrative, and EH D's Spill Response Authority Summary, per her <br /> request. <br /> 01/06/2021 Reviewed and completed complaint documentation revisions from Elena Manzo, FLORIDO <br /> Ld Sr REHS.
