Complaint Investigation Summary Report*5105 <br /> COMPLAINT ID: COGLRUWXG Site Location: NEAR 5151 99 FRONTAGE RD <br /> Activity Summary <br /> Activity Date Recorded by <br /> 09/03/2020 5 AM - 7 AM: Received complaint from SJC General Hospital. Called and spoke to FLORIDO <br /> Harleigh Overton of PG&E who states an unknown amount of non PCB oil spilled <br /> onto the roadway due to a vehicle impacting a pole-mounted transformer; a <br /> maximum of 18 gallons could have spilled; no drains impacted; PG&E is working <br /> on getting traffic control; the clean-up contractor's, PSC, ETA isβ2 hours; he does <br /> not currently have the PSC contact information;Adrian Delgado (209-405-1690) <br /> will be the on-site PG&E contact who will be repairing the transformer. Called and <br /> spoke to Adrian Delgado who states he is not on scene yet, he does not have the <br /> PSC contact information, and referred me to his supervisor, Jimmy Patterson <br /> (209-610-8897).Called and spoke to Jimmy who states he does not have the PSC <br /> contact info. Harleigh provided me with the spill clean-up crew forman contact info: <br /> Chris Feathers (530-228-2040), who is actually with Patriot Environmental, not <br /> PSC. <br /> Generated complaint and prepared for inspection. Called and spoke to Chris <br /> Feathers who states he is on-site and is going to begin the clean-up. Left home for <br /> site. <br /> Arrived on-site and met with Adrian Delgado of PG&E and Chris Feathers of <br /> Patriot. Observed a 5x20ft spill of transformer oil across the two lane roadway. A <br /> 1x1ft area of soil was impacted immediately beneath the transformer, located on <br /> the east side of the road. Took photos. Patriot to pressure wash the road, <br /> excavate the impacted soil, and scrap up the vegetation around the transformer. <br /> Chris confirmed a sample of oil from the affected transformer was taken, which is <br /> to be analyzed for PCBs. Chris to provide the EHD with a copy of the oil sample <br /> chain of custody, and the amount of waste generated and where it will be <br /> transported to, via the completed waste shipping paper. Documented narrative. <br /> Left site for EHD. Note: My time spent driving to the spill and then to the EHD is <br /> not included in this complaint time, since it is my typical drive to work. <br /> 09/03/2020 Received complaint information from Elianna Florido (EHD).
