Complaint Investigation Summary Report*5105 <br /> COMPLAINT ID: COGLRUWXG Site Location: NEAR 5151 99 FRONTAGE RD <br /> Activity Summary <br /> Activity Date Recorded by <br /> 09/03/2020 7:55 AM Received call on the ER Cell Phone from Chris Feathers (Patriot MURO <br /> Environmental Services Special Operations Hazmat Supervisor) 530-228-2040. <br /> He wanted to let us know they will be done with the cleanup in about 30 minutes. I <br /> let him know I will be heading out to verify the cleanup. <br /> 8:05 AM Left EHD. <br /> 8:21 AM Arrived at the PG&E cleanup site which was under traffic control by <br /> PG&E. Met with Chris Feathers. Chris Feathers told me they applied Simple <br /> Green to the street and then power washed. The rinsate was collected. They <br /> repeated this process three times. All rinsate from the triple wash was collected <br /> and placed into a 55-gallon drum labeled as Non-RCRA Hazardous Waste Oily <br /> Liquid. <br /> The contaminated soil from both shoulders of the road was removed and placed <br /> into a 55-gallon drum labeled as Non-RCRA Hazardous Waste Soil and Debris <br /> contaminated with mineral oil. <br /> Observed the curb on the side of the road was oily. I asked Chris Feathers if it was <br /> cleaned, he reported it was not, but he will wipe it down with rags as soon as he is <br /> done with what he was doing at the moment. I asked him to send me a photo once <br /> the curb was wiped off. Aside from that, no oily sheen or odor was observed on <br /> the street and soil. <br /> One 55-gallon drum of oily liquid waste was generated. One 55-gallon drum of oily <br /> solids waste was generated. All waste will be taken to 4040 West Lane, Stockton, <br /> CA 95204 pending analytical lab for waste disposal. <br /> Received the photo from Chris Feathers via text. The curb cleanup was adequate. <br /> Cleanup on the street, soil, and curb was adequate. <br /> 9:01 AM Left site. <br /> 9:17 AM Arrived at EHD. <br /> Worked on the Photo Log from today's cleanups. <br /> Entered narrative. <br /> 09/08/2020 Reviewed and revised narrative. Completed and submitted ER OT paperwork. FLORIDO <br /> 11/18/2020 Reviewed narrative. Emailed Alex Steele requesting he provide the PCB analytical FLORIDO <br /> results, quantity of waste generated, and location where the waste was <br /> taken/disposal record. Called Chris Feathers, Patriot Environmental on-site <br /> clean-up person, but was unable to leave him a voice message. Sent Chris a text <br /> message requesting him to provide the amount of waste generated and the <br /> location where the waste was taken. <br /> 11/18/2020 Revised narrative. Printed and organized complaint documentation according to FLORIDO <br /> new complaint abatement procedures. Emailed Claudia Muro, EHS, requesting <br /> she provide me with her photo log for this complaint. <br /> 11/19/2020 Reviewed PCB analytical received today from Alex Steele via email. PCB FLORIDO <br /> analytical results show that the oil is non-detect for PCBs. Alex states two drums <br /> of waste was generated and taken to the PG&E facility at 4040 West Ln. in <br /> Stockton. <br /> 11/20/2020 Received Request from Elianna Florido for photo log. MURO <br /> 12/01/2020 Emailed the cleanup photo log to Elianna Florido. MURO <br /> 5105.rpt Page 3 of 4 <br />
