Complaint Investigation Summary Report#:5105
COMPLAINT ID: CON07DK5W Site Location: 1000 E ROTH RD
Activity Summary
Activity Date Recorded by
06/01/2020 Sent an email to Paul Holt<pholt@up.com> and Michael J. Algots ALANIZ
<mjalgots@up.com>, asking for an update.
06/04/2020 Received disposal records from Derek Smith, Patriot Environmental, ALANIZ
<dsmith@patriotenvironmental.com>, on 6/4/2020. Sent a reply back asking for
waste determination documents.
06/05/2020 Received a call from Paul Holt, 562-490-7020. He said he would have the ALANIZ
analytical sent to me.
07/10/2020 Sent an email to Derek Smith <dsmith@patriotenvironmental.com> asking for the ALANIZ
waste profile.
08/10/2020 Answered billing questions for accounting. ALANIZ
10/12/2020 Reviewed complaint and still need to received the analytical results. Sent an email ALANIZ
to Michael J. Algots <mjalgots@up.com> and Paul Holt <pholt@up.com> asking
for the analytical results.
10/22/2020 Received the analytical results from Paul Holt, <pholt@up.com>, on 10/19/2020. ALANIZ
Paul stated:
I spoke with Patriot who handled the clean up for LOUP and found they used an
SDS and the fact that the label of the lube oil said non haz to determine the non
haz status of the waste. I have attached the SDS for the lube oil and a picture of
the can that was the cause of the release. As for the sausage casings there is no
waste determination for the food grade consumables, and the rags that were used
to wipe up the liquid were placed in the same drum as the lube oil.
Based off of the SDS and the statement received, there were no hazardous waste
concerns regarding fish toxicity, pH, or heavy metals. Approximately 60 pounds of
waste was generated and transported on a non-hazardous waste manifest to
Crosby and Overton in Long Beach.
Gathered the complaint documents together and abated the complaint. Gave to
supervisor for review.
12/14/2020 Corrected complaint information. Re-printed narrative and complaint cover sheet. ALANIZ
Gave to lead senior for review.
5105.rpt Page 4 of 4
