Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: CON07DK5W Site Location: 1000 E ROTH RD <br /> Activity Summary <br /> Activity Date Recorded by <br /> Calabro informed me that Meyer preferred email. I was informed that Meyer <br /> would be emailing me and that he would be my contact with Loup going forward. <br /> Regarding release 2 with the battery acid and the hydraulic oil: Oily staining was <br /> observed on the side of car EMHU 249268. Oily staining was also observed on the <br /> underside of the car containing the released material (EMHU 263361).According <br /> to Derek Smith, Field Supervisor, no fluid was observed around the battery. Only <br /> one gallon of hydraulic oil was released from this incident. The oil was cleaned up <br /> using absorbent and any contaminated containers were wiped down. One blue 55 <br /> gallon poly drum and one white 5 gallon bucket of oily debris was generated. They <br /> were labeled as hazardous waste but are going to be profiled for proper disposal. <br /> Created the complaint in ECR. The notice to abate was written and emailed to <br /> Calabro at StreamlineOperations@up.com. <br /> Left the site at 5:05 p.m. and arrived back home at 5:48 p.m. <br /> Paul Holt's Email: <pholt@up.com> <br /> 08/26/2019 Received a confirmation email from Chris Calabro, ALANIZ <br /> Stream]ineOperations@up.com, saying he had received the notice to abate. <br /> Received two spill reports. Filed them with the complaint. <br /> Downloaded the photos and created a photo log. <br /> Completed the Prop 65 and the ER record. Filled in the geo tracker coordinates. <br /> Looked up property owner address on parcel viewer. <br /> Completed the narrative from 8/25/2019. <br /> Received several discussion emails between Paul Holt pholt@up.com and Loup. <br /> At 1:45 p.m., Holt wrote: Patriot has completed the rework and blocking and <br /> bracing. Both containers have been released. <br /> 10/02/2019 Sent an email to <StreamIineOperations@up.com> (responsible party) asking for ALANIZ <br /> an update. Cc'd Paul Holt, Environmental Manager. Received an update from <br /> Patriot Environmental contact, Joseph McGuffin at <br /> <jmcguffin@patriotenvironmental.com> letting me know that he would be sending <br /> me the final disposal records. <br /> 10/03/2019 Received an update email from Michael J. Algots <mjalgots@up.com>, Union ALANIZ <br /> Pacific(UP) Environmental. UP hires GHD to analyze sample results. Michael <br /> asked GHD to provide an update. Olivia Deck <Olivia.Deck@ghd.com>, Waste <br /> Coordinator, replied "Sampling plans were sent to Patriot on 9/16/19. We are <br /> waiting on analytical for this waste." <br /> 11/08/2019 Received a call from Anfleigh Arbashor, 402-544-3249, arbashor@up.com asking ALANIZ <br /> about the invoice and wanted to know the car numbers for billing purposes. Gave <br /> her the information. <br /> 01/29/2020 Received billing questions from accounting and Mike Algots, <mjalgots@up.com>. ALANIZ <br /> 01/30/2020 Received a call from Joseph McGuffin, Emergency Response Manager, ALANIZ <br /> 916-256-4914, regarding waste generated from this event. He said that the waste <br /> was still at the rail yard and that he was in the process of getting rid of it. The oily <br /> debris is going to be sampled. Joseph is going to try to find the SDS for the salt <br /> brine casing to prove it is non-hazardous. If he cannot find it, he is going to send <br /> me a disposal record. <br /> 02/03/2020 Received emails from Daniel J. Meyer with Loup <djmeyer@up.com> and Paul ALANIZ <br /> Holt<pholt@up.com> regarding where the waste was. Daniel had some <br /> confusion. Problem sorted. Also, Joseph McGuffin does not work with Patriot <br /> Environmental. The new person is Derek Smith <br /> <dsmith@patriotenvironmental.com> <br /> 5105.rpt Page 3 of 4 <br />
