Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: COSZGWLWK Site Location: 2357 PISA CIRCLE, STOCKTON, CA <br /> Activity Summary <br /> Activity Date Recorded by <br /> 11/21/2020 8:56 am received a page from the SJC General Hospital. Called General Hospital MANZO <br /> and obtained PG&E transformer spill information for a residential location at 2357 <br /> PISA Circle, Stockton, CA. Mike Green 925-658-0437 has reported to the <br /> hospital operator that they had a transformer spill and that it was already cleaned <br /> up, and that the pad-mounted transformer was already replaced. I checked the <br /> email for the spill report, two spill reports were received, one at 4 am, and one at <br /> 4:14 am—the initial spill report(Cal OES Control#:20-6501) and the update. Per <br /> the spill reports: "A vehicle struck a pad-mounted transformer causing the release, <br /> material flowed onto the soil, vegetation, concrete and asphalt, RP is handling the <br /> containment and clean up. " <br /> I called Michael Green at 925-658-0437; Mike indicated that the spill happened <br /> last night, a pad-mounted transformer was hit by a vehicle in a residential area of <br /> Stockton. Mike stated the transformer had been replaced, and Dave Larsen of <br /> PG&E conducted the cleanup. Mike noted that approximately 30 gallons of <br /> non-PCB transformer oil was released from the pad mount transformer. Some <br /> released oil went onto a residential lawn, and some flowed onto the street and <br /> pavement area. No storm drains were impacted. Mike indicated that the spill had <br /> occurred at 3:50 am this morning and that he called to notify the EHD at that time <br /> (the call was forwarded to the SJC General Hospital). Mike indicated that he called <br /> 209-468-3420, and the General Hospital on-call service did not pick up. Mike <br /> indicated that he made the Cal OES notification and then attempted to call again <br /> this morning to notify the EHD again. Mike indicated that, this morning he was able <br /> to get through, and that's how the EHD was paged. <br /> 9:12 am called Dave Larsen with PG&E 209-401-9719, the mailbox was full, and I <br /> could not leave any messages. <br /> 10 am-arrived on-site observed PG&E crew and crane working on replacing the <br /> transformer. Met with Dave Larsen with PG&E. Dave indicated the cleanup had <br /> not been completed yet. Twelve drums (55 gallons each) of excavated soil and <br /> used absorbent were generated. Dry absorbent was applied to the stained <br /> pavement area adjacent to the steel site and swept up. All generated waste drums <br /> were labeled with hazardous waste labels. An area of several feet wide and <br /> several feet deep was excavated around the transformer pad. The lawn area <br /> appeared slightly wet; no odor or oily sheen was observed on the grass in the area <br /> of the transformer. Dave Larsen indicated house residents were made aware of <br /> the incident; one of the residents is currently home sleeping after a night shift. <br /> Dave indicated that he provided the resident with his contact information for lawn <br /> repairs. Dave indicated that an oil sample was collected for PCB determination. <br /> Dave also indicated that several hundred households are still without power since <br /> yesterday, and they are making power restoration their priority. Since the resident <br /> was sleeping, I was not able to make contact with the property owner. <br /> 10:15 am called Mike Green and let him know that the transformer has not been <br /> replaced yet, and the cleanup has not been completed. Mike appeared to be <br /> surprised to hear this information. He asked if Dave received the sample results <br /> back. I told him that he did not indicate to me that he did or not. I asked Mike <br /> about the property owner's contact information. Mike indicated that Alex Steele <br /> would be the best person to contact regarding this information on Monday. <br /> According to Dave Larsen, the spill occurred between 9 pm and 10 pm last night. <br /> Dave also indicated that he let Mike with PG&E know regarding the spill at that <br /> time it happened on 11/20/2020. Dave indicated that sprinkles were on for <br /> watering sometime before or after the spill had occurred. Prior to EHD's arrival, <br /> the concrete pad under the transformer was removed, and the soil under the pad <br /> was excavated. Dave indicated that he removed 12 drums of soil from under the <br /> transformer pad and that all visible contamination was excavated. He will provide <br /> pictures of the spill itself as well as the cleanup before the new pad was installed. <br /> Since EHD was not notified when the spill occurred, I was not able to verify the soil <br /> condition below the replaced concrete pad.
