Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: COSZGWLWK Site Location: 2357 PISA CIRCLE, STOCKTON, CA <br /> Activity Summary <br /> Activity Date Recorded by <br /> excavation site around the transformer pad. Dave Larsen indicated that he <br /> excavated more than he thought was necessary below the transformer pad "just to <br /> make sure", and he will provide confirmation pictures. No evident staining was <br /> noted on excavated dirt area around the newly installed concrete pad; stained <br /> pavement and gutter area were cleaned using dry absorbent. Verified that the <br /> nearby storm drains were not impacted by this spill, clean up appears to be <br /> adequate. Sent a text message to Dave Larsen asking to provide a copy of waste <br /> shipping documentation and clean up pictures. <br /> 2 pm-generated a complaint, looked up Parcel Viewer property information, <br /> generated notice to abate, and emailed the notice to Alex Steele, Steele, Rodney <br /> <R2Sv@pge.com>. Generated ER record. Generated prop 65 and emailed to G. <br /> Martinez (EHD)for processing, completed the narrative. <br /> (8:56 am-1:38 pm) <br /> 11/23/2020 Finalized complaint narrative. Called Alex Steele to inquired about excavation MANZO <br /> pictures and sample results. No answer, left a voice message. Downloaded <br /> inspection pictures, created picture file. <br /> 11/24/2020 Called Alex Steele with PGE, tried both numbers, 209-736-6426 and MANZO <br /> 209-337-337-8902, no answer, left a voice message asking for PCB lab result <br /> copy. Provided my email address. <br /> 11/30/2020 Sent a message to Dave Larsen PGE (209-401-9719) asking for shipping MANZO <br /> document, pictures and lab results. Received a text message back indicating that <br /> Dave is on vacation, and to contact Alex Steele. Responded with <br /> acknowledgment. Sent an email to Steele, Rodney <R2Sv@pge.com> asking <br /> again for above information. Received read receipt conformation from Alex. <br /> 12/09/2020 Reviewed an email received from Steele, Rodney <R2Sv@pge.com> on MANZO <br /> 11/30/2020 which included a copy of the shipping paper, laboratory analytical <br /> results showing ND- non detect for PCBs and pictures from the excavation <br /> progress. Printed the attachments. Sent an email to Alex asking for a picture <br /> showing the final clean up before transformer pad was installed, since EHD was <br /> not notified and was not able to verify clean up below the installed pad (only the <br /> area of the pad was visible at the time of the EHD inspection). <br /> 12/10/2020 Received an email from Alex Steele, Steele, Rodney <R2Sv@pge.com> indicating MANZO <br /> that he doesn't have a picture of the final clean up, sent a text to Dave Larsen, <br /> 209-401-9719, asking for a picture of the final clean up. <br /> 02/05/2021 Organized complaint documentation, printed ER record, created a site map, called MANZO <br /> Dave Larsen at 916-897-6779, Dave indicated that he provided all the excavation <br /> pictures that he had to Alex Steele and that he doesn't have any additional <br /> pictures, Dave confirmed that all the impacted soil below the transformer was <br /> excavated as evidenced by the 12x55-gallon drums removed from below the <br /> transformer pad. Made a note under PR0513829 for the generated waste. Abated <br /> the complaint. <br /> 5105.rpt Page 3 of 3 <br />
