Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: COTJ202RZ Site Location: SMITH CANALAT SHIMIZU DR & OCCIDENTALAVE <br /> Activity Summary <br /> Activity Date Recorded by <br /> 10/06/2020 Received an email from Ajay Powell, City of Stockton Public Works Field FLORIDO <br /> Specialist, stating Sean Alexander Marine Services will be removing the porta <br /> potty from Smith Canal this afternoon. Researched and called Sean Alexander <br /> Marine Services and spoke to Rob De La Cruz, who stated he will be the person <br /> removing the porta potty. Discussed complaint with Melissa Nissim, CUPA PC, <br /> and Elena Manzo, Ld Sr REHS. Called Rob back and he told me to be on-site at <br /> 1:30pm if I'd like to verify the removal of the porta potty. <br /> Arrived on-site and met with Rob and Ajay. Rob hadn't found the porta potty. I <br /> showed him where I had seen the porta potty, and we found that it was still tied to <br /> a tree. Witness Rob remove the porta potty from the canal. Rob determined there <br /> was still sewage inside the 10-15 gallon reservoir inside the porta potty. Rob <br /> stated he will take the porta potty back to his facility until the sewage can be <br /> removed, and once the sewage is removed he will dispose of the porta potty at a <br /> transfer station. After a few phone calls Rob's office assistant notified me that a <br /> port potty company called HoneyBucket will be removing the sewage from the <br /> porta potty tomorrow. I requested Rob's office assistant to provide me a copy of <br /> the HoneyBucket invoice and a disposal record/weigh bill for the disposal of the <br /> porta potty. <br /> Arrived home. Downloaded inspection photos and generated photo log. <br /> 10/07/2020 Completed 10/6/20 narrative and updated Elena Manzo and Melissa Nissim on the FLORIDO <br /> complaint. <br /> 10/07/2020 Emailed Robert McClellon and Natalia Subbotnikova in the Solid Waste Unit FLORIDO <br /> notifying them of this complaint. <br /> 01/19/2021 Reviewed narrative. FLORIDO <br /> 01/19/2021 Reviewed HoneyBucket invoice for the pumping of the porta potty and the disposal FLORIDO <br /> receipt for the disposal of the porta potty, received on 10/21/2020 from Chey Anne <br /> Peil, office assistant for Sean Alexander Marine Services Inc. <br /> 01/20/2021 Reviewed, printed, and organized complaint documentation and submitted to FLORIDO <br /> Elena Manzo, Ld Sr REHS for abatement. <br /> 5105.rpt Page 4 of 4 <br />
