Joe Grindstone Association Page 6 of 6 ' <br />13550.Highway 12, Lodi <br /> <br />January 29, 2019 <br />000048503 <br />Destroy the abandoned well under EHD permit. <br />Submit an application and Technical Report to operate a public water system to the <br />State Water Resources Control Board, Division of Drinking Water.. <br />Submit an engineer-designed onsite wastewater treatment system (OVVTS) permit <br />application that meets the requirement of EHD. <br />Obtain an annual EHD permit required to operate the OVVTS. <br />Correct all other cited Violations and final all necessary permits required to correct the <br />violations. <br />Objections to these findings of violations may be appealed. In order to be valid, a written <br />appeal must be received by the EHD within thirty (30) calendar days of receipt of this <br />notice.
