The permit has been approved with the following <br />conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br />any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br />commencing work. All new addenda supersede the previous ones. <br />3. Provide written notification to ustt�slgov.orq to arrange the following inspection appointments <br />(a 48-hour notice is required for inspection appointments). <br />a. Corrosion protection: Verify corrosion protection for direct bury spill bucket prior to <br />backfilling. <br />b. Spill bucket test: Verify minimum 5 -gallon capacity and pass a one hour hydrostatic test (or <br />equivalent). Proper drainage must also be verified. <br />c. Since concrete will be broken near the diesel tank, a secondary containment test must be <br />performed on the diesel tank once concrete is poured in accordance with the <br />manufacturer's guidelines. <br />Please be aware that the facility cannot receive fuel in the diesel tank prior to the EHD <br />inspection and approval. <br />4. Any individual performing the work of a service technician must meet the following <br />qualifications: <br />a. Possess or be employed by a person who possesses one of several permissible valid, <br />current contractor's licenses issued by the Contractors State License Board (may be issued <br />to the installation company), or a Tank Tester's license issued to the individual by the State <br />Water Board <br />b. Possess or work under the direct and personal supervision of an individual physically <br />present at the work site who possesses a valid, current California UST Service Technician <br />certificate issued by the International Code Council (ICC) <br />1868 E.
