Safe Work Plan — JSA <br /> r <br /> Shell Stockton Terminal <br /> Job Tasks Outlined in Exhibit A - Shell Scope of Work Rev . A dated 7/22/2019 <br /> Scope of Work : <br /> Abandon existing contact water tank UG-5 in place <br /> • Triple clean tank <br /> • Have marine chemist verify the tank is clean <br /> • After confirmation the tank is clean then fill the tank with flowable fill slurry ( low strength lean concrete ) <br /> There are no expected Simultaneous Operations ( SIMOPS ) occuring in the vicinity of this work zone . <br /> SECTION 3 * SAFETY PROCEDURESAND POLICIES <br /> RequirementsGeneral <br /> The Foreman will be responsible for ensuring : <br /> 2 . All approved permits are displayed on the job site and requirements are adhered to as appropriate . <br /> 3 . Daily Tailgate meetings address all expected job site hazards by reviewing JSAs, confirm fitness for duty, and reinforce <br /> the support for Stop Work Authority . <br /> 4 . Verify all site workers are experienced and have appropriate training (API , Shell PI / PH , and Terminal training ) <br /> 5 . Verify length of service for each crew worker and implement the SSE program if any are present . <br /> 6 . Verify shaded break area and discuss breaks . <br /> .fecs-onad Protective . �, • . . <br /> PPE will be provided to work crew at no cost to them . Basic PPE for this work scope consists of: <br /> Level D — Hard hat, steel -toed boots, approved eye protection , reflective traffic safety vest, and appropriate gloves when needed . <br /> It is not expected that respirators will be required . If hydrocarbon vapors are measured in the breathing zone, work will stop and <br /> a plan to eliminate the hazard will be implemented . <br /> The Foreman will ensure that all crew members have the necessary PPE prior to the start of work and will advise the crew to <br /> practice positive intervention . <br /> PPE will be inspected prior to use and replaced if needed . <br /> Foreman will coordinate the set up an exclusion zone using caution tape and barriers to keep unauthorized people out of the <br /> work area . All equipment not needed for the work scope should be parked outside the exclusion zone . <br /> The area around the work zone will be kept clear. Hoses and cords will be kept out of walkways and working surfaces . <br /> WASTE MANAGEMENT <br /> The task of tank cleaning will generate water mixed with gasoline . The waste mixture will be transferred directly into the vacuum <br /> truck during the cleaning operation . The waste mixture will be removed from the site at the end of the work day . <br /> SPILL CONTROL <br /> There is a potential for a liquid spill on site should a hose or fitting leak .
