This information must be kept readily accessible at all times for the duration of the job in the event of an emergency. <br /> The foreman is also responsible for following any customer-specific requirements relative to injury reporting and response . The foreman will be <br /> trained in First Aid so that educated decisions are made when assessing the severity of the situation following an incident or injury. <br /> DEFINITIONS <br /> 1 . Critical Incident - An incident resulting in serious injury where a potentially life-threatening situation is present, including ( but not limited <br /> to ) the following scenarios : <br /> a . Loss of consciousness or impaired consciousness <br /> b . Significant loss of blood or inability to control bleeding <br /> C , Head, neck or spinal injury ( or potential head, neck or spinal injury) <br /> d . Shock <br /> e . Shortness of breath or trouble breathing, Chest pain <br /> g . Obvious broken bones or fractures making it difficult to remove the victim from the scene <br /> A call to 911 should be made immediately for all Critical Incidents . <br /> 2 . Near Miss - An event that consists of a combination of unsafe conditions, behaviors and hazards that could have resulted in an injury but <br /> - due to circumstances outside of our control - did not result in an injury. <br /> 3 . Non-CriticolIncident - An event that results in injury that is not life-threatening ( i . e . , pinched finger, foreign object in eye or minor <br /> laceration where bleeding is controlled ) but still necessitates medical attention . <br /> 4. Positive Intervention — A condition recognized as unsafe where a crew member intervenes to correct the unsafe condition . <br /> 5 . Stop Work Authority — Given to all persons on the work site to authorize them to stop work and speak up about any act or condition they <br /> observe to be unsafe . <br /> PROCEDURE : Follow the Q$ EMS Event Communication Flowchart ( next page ) <br /> 1 . Response to the injured employee ( s ) is the primary function following any significant incident <br /> a . For critical incidents, the injured employee should be transported to the ER via ambulance ( 911 call ) <br /> St .
