Within 72 hours of being provided a signed copy of the report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the required time frame and document follow-up action to all compliance issues in the future. This is a repeat violation, Class II. 209 CCR 2715(c)(3), 2715(c)(4) Failure to maintain a copy of the"Facility Employee Training Certificate"on-site. OBSERVATION: The Facility Employee Training Certificate was not signed by the designated operator and a date of training was not documented. REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a repeat violation, Class II. 301 HSC 25292.1(a)Failure to operate UST system to prevent unauthorized releases including leaks,spills and overfills. OBSERVATION: The fuel filters under the dispensers are not located to allow drips and leaks to drain back into the UDC. REGULATION GUIDANCE: The underground storage tank system shall be operated to prevent unauthorized releases. CORRECTIVE ACTION: Immediately provide approved secondary containment for the filters under permit and inspection by the EHD. This is a Class II violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by April 30, 2021 . To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of having them sign.
