S A N J 0 A Q I I I Irl Environmental Health Department <br /> C o u N T AMENDED <br /> 4/28/2021 <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> FLAG CITY CHEVRON 6421 CAPITOL AVE, LODI April 26, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 306 HSC 25291(e) Failure to remove, analyze, and properly dispose of water in secondary containment(pre-Jul 2003). <br /> OBSERVATION:Approximately 2-3 gallons of liquid was observed in the 87(main),diesel, 91 and 87 (siphon)fill <br /> sumps. <br /> REGULATION GUIDANCE: If water could enter into the secondary containment by precipitation or infiltration, it <br /> must be removed and disposed of properly. <br /> CORRECTIVE ACTION: Immediately remove this liquid, make a hazardous waste determination per Title 22 <br /> hazardous waste regulations, and manage it accordingly. Ensure that all sumps and annular spaces are maintained <br /> free of liquid. <br /> This is a repeat violation, Class ll. <br /> 318 CCR 2635, 2637.1,2665, 2712 Failed to meet spill container requirements. <br /> OBSERVATION: The 87(main)spill container failed when tested. <br /> REGULATION GUIDANCE:All spill containers shall have a minimum capacity of five gallons and be capable of <br /> containing a spill or overfill until it is detected or cleaned up. <br /> CORRECTIVE ACTION: Immediately contact a properly licensed,trained, and certified contractor to repair or <br /> replace the spill container under permit and inspection of the EHD. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator on the day of inspection. A complete report was <br /> finished on April 28, 2021, and replaces the initial checklist. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by May 28, 2021. <br /> Please note that violation 311 was removed and 306 was added.
