SAN J 0 A Q I I I Irl Environmental Health Department <br /> C o u N T AMENDED <br /> 4/28/2021 <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> FLAG CITY CHEVRON 6421 CAPITOL AVE, LODI April 26, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 205 CCR 2712(i) Failure to have current UST Monitoring Plan readily accessible at the facility. <br /> OBSERVATION:An approved copy of the monitoring plans were not available for inspection. <br /> REGULATION GUIDANCE:A copy of these plans shall be accessible on site at all times. <br /> CORRECTIVE ACTION: Immediately locate and retain a copy on site. <br /> This is a Class II violation. <br /> 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. <br /> OBSERVATION: The designated operator failed to check and note all required items on the visual inspection <br /> reports.The incorrect and missing information included the following from the inspection reports: <br /> - Incorrect monitoring system certification date <br /> -The operator of the UST system failed to sign the report <br /> -Section 5 was left blank <br /> -The incorrect designated operator report was used for the 10/12/2020 report <br /> REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> This is a repeat violation, Class Il. <br /> 302 CCR 2630(d), 2641(a)Leak detection equipment not installed/maintained/located to detect leaks at earliest <br /> opportunity. <br /> OBSERVATION: The 91 grade annular sensor string broke as the technician was putting the sensor back into <br /> place.
