This information was required to be submitted by <br /> 6/19/2020 and was received on 7/13/2020. <br /> REGULATION GUIDANCE:An operator that receives an inspection report shall have 30 days to implement the <br /> corrections specified in the inspection report and comply with permit conditions. The corrective action shall include <br /> all of the recommendations made in the report. The operator shall submit a written response that includes a <br /> statement documenting corrective actions taken or proposing corrective actions which will be taken. <br /> CORRECTIVE ACTION: Ensure that a written response documenting corrective actions taken or proposed is <br /> submitted and all corrections implemented within 30 days of receiving an inspection report. <br /> This is a repeat violation, Class II. <br /> 203 CCR 2712(b)(1-2)Failure to maintain monitoring records for 36 months and maintenance records for 60 months. <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. The facility <br /> did not have a work order for a dispenser 1-2 fuel alarm (L13)that occurred on 4/18/2021. The manager, Kumars <br /> Khajevardi, stated that a contractor is called when an alarm goes off. He stated that this alarm was addressed but <br /> could not provide any documentation. <br /> The facility did not have overfill test results for the 87 grade(main), 87 grade(siphon), and diesel tanks. <br /> REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records <br /> include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance <br /> records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs <br /> of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of <br /> inventory readings and reconciliations. <br /> CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the <br /> last three years on site and submit copies to the EHD. <br /> This is a repeat violation, Class II. <br /> FA0000485 PR0231706 SCO01 04/26/2021 <br /> EHD 23-01 Rev.9/16/2020 Page 4 of 6 Underground Storage Tank Program OIR <br /> 1868 E.
