California Hazardous Waste Disposal Guidelines <br />Job Aid <br />5 <br />Complete the Hazardous Waste label and attach it to the box. <br />Jjr <br />• Shipper: CARQUEST Auto Parts,rw$H„.��n <br />HAZAROO 1�iA5TE <br />• Address: Store street address <br />5 <br />• City, State, Zip: Store City, State, Zip <br />• EPA ID No.: Contact Environmental Affairs <br />—�- <br />• Accumulation Start Date: Do Not fill in the date until a waste^ <br />item is placed into the box. <br />Hniaat w cisn� <br />Disposal and Donation Frequency and Transport <br />Disposal must be performed when the Hazardous Waste tote is nearly full or as appropriate. <br />• Regardless of whether the hazardous waste tote is full or not, it must <br />be disposed of 6 months after the first <br />item was added to the box to stay in compliance. <br />• Products must be secured in the tote and self -transported to a local <br />household hazardous waste facility. <br />o If needed, add empty cardboard to the box to keep items <br />from shifting. <br />o Strap the box in place so it does not slide during transport. <br />Note: If the amount to be disposed of is more than 200 lbs. or 27 gallons, you <br />must contact Environmental Affairs at 540- <br />561-8492 for disposal instructions. <br />Hazwaste Document Control <br />• Always update and maintain a Hazwaste Inventory List. <br />X =HASTE .rUry lW <br />fAuaN TJA4ZWASTF tsn rj Le M vWf14 <br />• When possible, obtain a receipt from the drop off location,•"~ <br />o Ensure the receipts contain the following info: <br />f GATE PART 01SC <br />OEAYTICN UOU QUANTITY <br />a ro1.ect„ � seeks �.� , <br />o Drop off location and facility name <br />o Name of disposer <br />o Date <br />Note: Some receipts may provide the amount being disposed of. <br />• For donated items, print an account invoice and have the customer or <br />Vo -Tech school representative sign and <br />date the ticket. <br />• Proof of disposal and donations must be filed as HW Documentation <br />and stored in an easily accessible location <br />in the manager's office for 3 years. <br />• Refer to Safety Data Sheets (SDS) to get important information about a chemical or product in your store before <br />attempting any clean up. <br />• In case of a fire, spill or other emergency involving hazardous chemicals or waste, refer to the Emergency <br />Procedures and Contingency Plan for the following: <br />o Major Emergency steps <br />o Minor Emergency steps <br />o Emergency coordinators (must be available 24 -hours to assist emergency response personnel) <br />o Emergency contacts and release reporting <br />o Emergency Equipment location <br />• Ensure all Team Members know the location of the document and are familiar with the policies and procedures. <br />Last updated: October 11, 2019 <br />®f9tJTOPRRT WORLDPAC.�.�iy�.
