According to the Ulta Potentially <br /> Hazardous Waste Management poster, fragrances and fragrance testers are to be placed in a Flammable Liquids <br /> and Solids (FL) Hazardous Waste container. <br /> -A box of Eylur Naturals 031 Triple Pack with Lashfix Glue was identified as a Flammable Liquid and Solid on the <br /> scanner(IMG 8461-2). <br /> -Three bottles of nail polish. According to the Ulta Potentially Hazardous Waste Management poster, nail polishes <br /> are to be placed in a FL Hazardous Waste container. <br /> -A Mont Blanc Legend Night perfume (IMG 8462). <br /> -Three perfume testers (IMG 8462). <br /> 6. A container labeled as Flammable Liquid & Flammable Solid (FL) Hazardous Waste contained the following <br /> waste item (IMG 8463-4): <br /> -A 2.8 oz aerosol can of Morphe Continuous Setting Mist(IMG 8468-70).According to the Ulta Potentially <br /> Hazardous Waste Management poster, aerosol cans are to be placed in a FG Hazardous Waste container. <br /> REGULATION GUIDANCE: The business owner must ensure that all employees are thoroughly familiar with proper <br /> waste handling and emergency procedures, relevant to their responsibilities during normal facility operations and <br /> emergencies. <br /> CORRECTIVE ACTION: Immediately train the employees. Provide proof of training, and training materials, within <br /> 30 days to the San Joaquin County Environmental Health Department(EHD) for employees whose responsibilities <br /> include hazardous waste. <br /> This is a Class II violation. <br /> 109 CCR 66262.23(x)(4) Failed to send generator manifest copies to DTSC within 30 days. <br /> OBSERVATION: 43 generator copies of Uniform Hazardous Waste Manifest(UHWM)within the date range of <br /> 6/30/2018 through 10/01/2019 were not sent to the California Department of Toxic Substances Control (DTSC) <br /> within 30 days of hazardous waste shipment. At the time of inspection, Heather Dela Cruz, Co-Manager, stated that <br /> the facility does not mail a copy of the hazardous waste manifests anywhere, and has been directed to shred the <br /> hazardous waste manifest paper copies upon receiving the manifests from the hazardous waste hauler. <br /> REGULATION GUIDANCE: Hazardous waste generators are required to send a copy of the generator signed <br /> UHWM to DTSC by mail. The only exception is when an UHWM starts and finishes as an electronic manifest in the <br /> e-Manifest System. The generator shall within 30 days of each shipment of hazardous waste submit to DTSC a <br /> legible copy of each manifest used. The mailing address is: DTSC Generator Manifests, Department of Toxic <br /> Substances Control, P.O. Box 400, Sacramento, CA 95812-0400. <br /> CORRECTIVE ACTION: Immediately mail a copy of the UHWMs within the date range of 6/30/2018 through <br /> 10/01/2019 and ensure that copies of all generator signed UHWMs are sent to DTSC within 30 days of shipping the <br /> hazardous waste.
