S A N J O AN N Environmental Health Department <br /> AMENDED <br /> 10/9/2019 <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> ULTA BEAUTY#1152(Lodi) 1423 S LOWER SACRAMENTO RD, Lodi October 07, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 405 CCR 66262.34(d)(2) Failed to manage incompatible wastes in containers properly. <br /> OBSERVATION: The following incompatible wastes are stored in way that is not in compliance with the regulatory <br /> sections listed below: <br /> 1.The waste items listed below were found in a silver container observed in the Ulta Salon and labeled with the <br /> words"This trash can is for Salon color tubes only,""Empty Daily,""Hazardous Waste Satellite Accumulation Area," <br /> "Oxidizers: Liquids and Solids,"and with the generator name and address(see photo IMG 8539-42). The container <br /> also contained several bottles and tubes of hair color and hair developer containing hydrogen peroxide,which are <br /> classified as oxidizer hazardous waste according to the Potentially Hazardous Waste Management poster(IMG <br /> 8552-5). Christine stated the container is not being emptied daily by the Salon employees, since the container was <br /> over half-filled with waste items and the first salon employee for the day would be arriving at Spm on the date of the <br /> inspection. <br /> -A 4.0 oz aerosol can of Living Proof Perfect Hair Day dry shampoo(IMG 8543-4). <br /> -A partially filled 128 fl oz container of Barbicide(IMG 8545-6). However,the scanner identified the waste item as a <br /> flammable liquid and solid(IMG 8547). <br /> REGULATION GUIDANCE: <br /> Incompatible wastes, or incompatible wastes and materials shall not be placed in the same container, unless <br /> section 66265.17(b)is complied with. <br /> (b)Hazardous waste shall not be placed in an unwashed container that previously held an incompatible waste or <br /> material. <br /> (c)A container holding a hazardous waste that is incompatible with any waste or other materials transferred or <br /> stored nearby in other containers, or open tanks, shall be separated from the other materials or protected from <br /> them by means of a dike, berm,wall,or other device. <br /> 66265.17(b)Where specifically required by other sections of this chapter,the transfer,treatment, storage,or <br /> disposal of ignitable or reactive waste, and the mixture or commingling of incompatible wastes, or incompatible <br /> wastes and materials,shall be conducted so that it does not: <br /> (1)generate extreme heat or pressure,fire or explosion, or violent reaction; <br /> (2)produce uncontrolled toxic mists,fumes,dusts, or gases in sufficient quantities to threaten human health or the <br /> environment; <br /> (3)produce uncontrolled flammable fumes or gases in sufficient quantities to pose a risk of fire or explosions; <br /> (4)damage the structural integrity of the device or facility containing the waste;or <br /> (5)through other like means threaten human health or the environment. <br /> CORRECTIVE ACTION: Immediately make changes to the hazardous waste storage to eliminate incompatible <br /> waste storage. Describe how the waste storage has been changed to correct the violation.
