The containers
were:
1.A Flammable Liquid&Flammable Solid(FL)Hazardous Waste container, containing waste fragrances, nail
polishes, and flammable cosmetics, was missing the generator's complete address on the hazardous waste label
(see photos IMG 8502-3).
2.The following waste item was found inside a container labeled as State Managed Waste/Toxic(ST) Hazardous
Waste(HW)liquids and solids(IMG 8511-2):
-A box containing a green bottle of Clinique liquid facial soap, a pink bottle of Clinique clarifying lotion, and a yellow
bottle of Clinique moisturizing gel(IMG 8516).The box and the pink bottle of Clinique clarifying lotion labeled the
clarifying lotion as flammable(IMG 8517-8).When the box was scanned, the hand-held scanner identified the entire
box, including the clarifying lotion, as State Managed Waste. Christine stated all three bottles were placed in the ST
HW container, based on the information provided by the scanner.
Based on the waste item listed above, the hazardous property listed on the container's hazardous waste label is
incomplete/inaccurate.
3.The following waste item was found inside a container labeled as ST HW liquids and solids(IMG 8519-20):
-An 8 oz metal aerosol can of Kenra Nitro Memory Creme(IMG 8521-3).According to the Ulta Potentially Hazarous
Waste Management posters, metal aerosol cans are to be placed in the Flammable Gas Hazardous Waste
container. When the aerosol can was scanned, the hand-held scanner did not indicate which hazardous waste
container the waste item should be placed in(IMG 8524). Christine stated that items that are not designated a
hazardous waste category on the scanner will automatically be placed into a State Managed Waste/Toxic
Hazardous Waste container.
Based on the waste item listed above, the hazardous property and the physical state listed on the container's
hazardous waste label is incomplete/inaccurate.
4. The following waste item was found in a container labeled Battery Recycling Kit and observed on the floor next to
the hazardous waste storage area(IMG 8525-8528):
-A 5.39 oz container of Wella Eimi Shape Shift molding gum(IMG 8529-30). However, the hand-held scanner
identified the waste item as a corrosive acid(IMG 8531).According to the Ulta Potentially Hazardous Waste
Management poster, corrosive acid waste items are to be placed in a Corrosive Acid Hazardous Waste container.
The facility did not have a container designated for Corrosive Acid Hazardous Waste at the time of inspection.
Based on the waste item listed above, the container was missing the words"Hazardous Waste", name and address
of generator, hazardous properties, physical state, composition(contents), and accumulation start date.
5.The waste items listed below were found in a silver container observed in the Ulta Salon and labeled with the
words"This trash can is for Salon color tubes only,""Empty Daily,""Hazardous Waste Satellite Accumulation Area,"
"Oxidizers: Liquids and Solids,"and with the generator name and address(IMG 8539-42).The container also
contained several bottles and tubes of hair color and hair developer containing hydrogen peroxide, which are
classified as oxidizer hazardous waste according to the Potentially Hazardous Waste Management poster(IMG
8552-5). Christine stated the container is not being emptied daily by the Salon employees, since the container was
over half-filled with waste items and there were no salon employees working at the time of inspection.
