COMPLAINT INSPECTION REPORT <br /> 1:15 pm <br />12:39 pm <br />Time Out: <br />Time In: <br />Environmental Health Department <br />Name of Facility: MIKASA ASIAN BISTRO & SUSHI BAR Date: 04/06/2021 <br />Address: 2610 S TRACY BLVD, TRACY 95376 <br />Owner/Operator: MIKASA ASIAN BISTRO & SUSHI BAR Telephone: (209) 830-8288 <br />Program Element: 1600 - FOOD PROGRAM Complaint #: CO0053619 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />CO0053619 states: : "COMPLAINANT ALLEGES THEY FOUND A COCKROACH IN THEIR FOOD." <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodborne illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br /> #23 Rodents, Insects or Animals Inside Facility <br />OBSERVATIONS: During this date's inspection, two live cockroaches (one adult and one juvenile) were observed at the <br />shelving above the two food preparation sinks. One dead cockroach was observed at this area. One dead cockroach was <br />observed at the sushi bar. <br />Live insects were killed. Clean up dead insects and sanitize affected areas. <br />Contact your pest control company and request a service visit ASAP. <br />CALCODE DESCRIPTION: Each food facility shall be kept free of vermin: rodents (rats, mice), cockroaches, flies.( 114259.1, 114259.4, <br />114259.5) <br /> #35 Equipment/Utensils Approved and in Good Repair <br />OBSERVATIONS: The hoods and hood filters have dripping grease. Degrease, clean and sanitize hoods and hood filters by <br />2 weeks. <br />CALCODE DESCRIPTION: All utensils and equipment shall be fully operative and in good repair. (114175). All utensils and equipment <br />shall be approved, installed properly, and meet applicable standards.
