Complaint Investigation Summary Report* 5105 <br />COMPLAINT ID: C00052853 Site Location: 2121 ZUCKERMAN RD <br />Activity Summary <br />Activity Date <br />Recorded by <br />10/26/2020 Received Cal OES spill report from Ralph Roberts. MURO <br />Discussed with John Alaniz (EHD) and Lead Senior Elena Manzo (EHD). <br />I called Ralph Roberts to ask if this was for a new release. Ralph told me it was <br />from the same release, but had to make a new oes report because larger amount <br />and additional spill location was discovered. It was determined to that the spill <br />report was for the same incident received by John Alaniz. <br />10/27/2020 Received an update email from Ralph Roberts, <RPRb@pge.com>. The email <br /> <br />ALANIZ <br />contained more photos of the cleanup. <br />Called Ralph 209-323-9492. He said that the cleanup was still going on and he <br />did not have an estimated time of completion. <br />Left for the site and met with Ralph. The contaminated areas were box 7W, box <br />11W, cellar 11W, and cellar 17W. The boxes were cement structures that were <br />placed around piping outside of the cellars. The boxes had a dirt floor. <br />Each cellar had the hydraulic oil pumped out and was pressure washed. Both <br />cellar 11W and 17W were deemed clean. No hydraulic oil was observed in them. <br />Two 55 gallon drums of oily water was generated at cellar 11W and five 55 gallon <br />drums of oily water was generated at cellar 17W. <br />The box areas are still being cleaned. The cleanup crew is currently in the <br />progress of putting up shoring so they can dig deeper. At the time of inspection, <br />one 20 yard bin of oily soil had been filled and the cleanup crew was going to start <br />filling another one. <br />The oily water is going to be handled as non-RCRA hazardous waste. Ralph is <br />going to test the oily solids to determine if the waste is hazardous. All waste <br />containers (including the roll-off bin) had hazardous waste labels on them and they <br />were correctly filled out. <br />10/27/2020 Discussed complaint with Elena Manzo, and other on call staff. Sent an email to ALANIZ <br />on call staff who might respond to it. <br />10/28/2020 Reviewed complaint details emailed to me by John Alaniz, REHS, in case I have FLORIDO <br />to verify the clean-up. Forwarded the email to Jason Gaitan, REHS, and Claudia <br />Muro, EHS, in case either of them need to verify the clean-up. <br />10/28/2020 Let Ralph Roberts of PG&E (209-323-9492) to let me know when the clean-up is BAKER <br />complete so I can verify. <br />Checked in with Ralph Roberts on the status of the clean-up. He said that it will <br />not be done today because they need shoring boxes. <br />10/28/2020 Received email from Elianna Florido IREHSI, reviewed photos and site map for GAITAN <br />clean-up that may occur tomorrow 10-29-2020. <br />10/29/2020 Discussed complaint with Jason Gaitain, REHS, who will be verifying the spill FLORIDO <br />clean-up today if needed. <br />10/29/2020 Discussed complaint with Michelle Henry ILead Senior REHSI GAITAN <br />5105.rpt <br /> Page 2 of 4
