Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COGBANCPA Site Location: 720 B. ST. <br />Activity Summary <br />Activity Date Recorded by <br />09/29/2020 Received email from Eddie Phillips, Maintenance Manager for BNSF. Attached GAITAN <br />were photos and a statement regarding lime powder. Stated that one 15 yard bin <br />of lime powder and track ballast were generated from waste clean up. Waste is <br />still on site pending on waste profile. Sent response email. <br />09/30/2020 Generated photo log from photos provided by Patriot Environmental. Left <br />voicemail for Derek Smith, supervisor for Patriot Environmental. Sent email to <br />Derek. Requested additional information on how far the lime powder was located <br />in the tracks and if any other methods were used to clean-up the lime powder. <br />09/30/2020 Received call from Isabella Bencomo, scientific aid for State Water Board. <br />Discussed complaint. She requested information regarding clean up. Stated that <br />clean up was already completed and that I was currently waiting for waste to be <br />disposed of. Sent Photos and notice to abate to Isabella's email. <br />10/05/2020 Received email from Derek Smith, Supervisor for Patriot Environmental. Stated <br />that clean up was performed at B-St. terminal out to Pilgrim St. Reached out to <br />James Farner, hazmat field manager. Asked about AIS role on clean up. Received <br />call from James discussed site clean up. Stated no lime powder was discovered <br />outside of B-St. terminal. Stated AIS was contracted to assist with clean up, they <br />checked the tracks after Patriot to ensure all lime powder was recovered. <br />GAITAN <br />GAITAN <br />GAITAN <br />10/05/2020 File review. GAITAN <br />01/11/2021 Consolidated ER Files, emails. Looked at DTSC site to find manifest for lime GAITAN <br />powder. <br />02/23/2021 Generated Confidential List and updated dailies. Sent email to James Farner, GAITAN <br />HazMat Field Manager, requested disposal receipt. <br />02/23/2021 Received email from James Farner, Haz Mat Field Manager, BNSF. GAITAN <br />Attached was disposal receipt for the soil impacted with lime powder. Accepted <br />manifest and attached to ER file. <br />03/08/2021 Reviewed ER file and printed out dailies and complaint investigation summary. GAITAN <br />Submitted ER file to Elena Manzo 'Lead Senior RENS'. Complaint closed. <br />5105.rpt Page 3 of 3
