Environmental Health Department <br />Hazardous Materials Program Compliance Review Report <br />Facility Name and Address:SVT Auto Service, 25533 N HWY 99 RD, ACAMPO 95220 <br />#8 HSC 25505(a)(4)Failed to establish and electronically submit a reasonable or an adequate training program <br />OBSERVATIONS:OBSERVATION:A training program has not been submitted. <br />REGULATION GUIDANCE:Training programs for all employees and annual training, including refresher courses, shall <br />include familiarity with the facility's Emergency response plans and procedures in the event of a release or threatened <br />release of a hazardous material.Training shall cover, but not limited to, all of the following: <br />(A)Immediate notification contacts to the appropriate local emergency response personnel and to the unified program <br />agency. <br />(B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, <br />property, or the environment. <br />(C)Evacuation plans and procedures, including immediate notice, for the business site. <br />These training programs may take into consideration of each employee. <br />CORRECTIVE ACTION:Immediately prepare or update the facility's training program, log into the California Environmental <br />Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for <br />approval. <br />This is a Class II violation. <br />#8 HSC 25505(a)(4)Failed to establish and electronically submit a reasonable or an adequate training program <br />#10 HSC 25508.1(a)-(f)Failed to electronically update the HMBP within 30 days of substantial change in <br />operations <br />OBSERVATIONS:OBSERVATION:An ownership change occurred on or about February 26,2021.The business has not <br />updated and submitted the information within 30 days. <br />REGULATION GUIDANCE:Within 30 days of one the following events, business shall electronically update the information <br />submitted to the California Environmental Reporting System (CERS): <br />-A 100 percent or more increase in the quantity of a previously disclosed material. <br />-Any handling of a previously undisclosed hazardous materials <br />-A change of business address, business ownership, or business name. <br />CORRECTIVE ACTION:Immediately log into the California Environmental Reporting System (CERS)at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />#10 HSC 25508.1(a)-(f)Failed to electronically update the HMBP within 30 days of substantial change in operations <br />Rev. 9/16/2020 Hazardous Materials Program Compliance Review Report <br />FA0010889 PR0520537 SC001 04/06/2021 <br />Page 3 of 5 <br />1868 E.Hazelton Avenue | Stockton, California95205 |T 209 468-3420 |F 209 464-0138 | www.sjgov.org/EHD
