Environmental Health Department <br />Hazardous Materials Program Compliance Review Report <br />Facility Name and Address:SVT Auto Service, 25533 N HWY 99 RD, ACAMPO 95220 <br />#11 HSC 25505(a)(4)Failed to provide or document initial and/or refresher training to appropriate personnel <br />OBSERVATIONS:OBSERVATION:Documentation showing initial and annual refresher training was not available at the time <br />of inspection. <br />REGULATION GUIDANCE:The business plan shall include provisions for ensuring that appropriate personnel receive initial <br />and annual refresher training.All employees shall be trained in safety procedures in the event of a release or threatened <br />release of a hazardous material, including, but not limited to, familiarity with the following plans and procedures: <br />(A)Immediate notification contacts to the appropriate local emergency response personnel and to the unified program <br />agency. <br />(B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, <br />property, or the environment. <br />(C)Evacuation plans and procedures, including immediate notice, for the business site. <br />This training shall be documented electronically or by hard copy and shall be made available for a minimum of three years. <br />CORRECTIVE ACTION:Immediately provide employee training for appropriate personnel and submit a copy of the training <br />records to the EHD. <br />This is a Class II violation. <br />#11 HSC 25505(a)(4)Failed to provide or document initial and/or refresher training to appropriate personnel <br />#12 HSC 25505(c)Failed to have the HMBP readily available to facility or EHD personnel <br />OBSERVATIONS:OBSERVATION:Facility could not provide the Emergency Response Plan and the Chemical inventory <br />upon request. <br />REGULATION GUIDANCE:The emergency response plans and procedures, the inventory of information required by this <br />article, and the site map required by this section shall be readily available to personnel of the business or the unified program <br />facility with responsibilities for emergency response or training pursuant to this section. <br />CORRECTIVE ACTION:Immediately make arrangements to have the business plan readily available for review. <br />This is a Class II violation. <br />#12 HSC 25505(c)Failed to have the HMBP readily available to facility or EHD personnel <br />OVERALL NOTES AND COMMENTS <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the <br />corrective actions that have been or will be taken for each violation, and any supporting paperwork, by May 6,2021. <br />Ownership change occurred on or about February 26,2021.On March 9,2021, business owner, Gary Hall was advised a <br />Hazardous Material Business Plan (HMBP)is due no later than March 28,2021.To date, there is no compliance activity on <br />record. <br />Starting September 1,2018, all in-office CERS help will be provided at EHD hourly rate ($152).
