Site Safety and Health Plan <br />Stockton Field Maintenance Shop #24 <br />8020 Airport Way, Stockton, CA <br /> <br />2.0 KEY PERSONNEL AND RESPONSIBILITIES <br />This section describes the personnel responsibilities for implementation of the SSHP. Clear <br />lines of authority will be established for enforcing compliance with the SSHP. <br />Designated Star personnel are responsible for field implementation of the SSHP. This includes <br />field supervision, maintaining contamination control zones, enforcing safe work practices and <br />decontamination procedures, ensuring proper use of PPE and communicating modified safety <br />requirements to site personnel. <br />To meet its safety and health objectives, the project team has developed a line of reporting and <br />has tasked individuals with the following safety and health responsibilities. <br /> <br />2.1 STAR PROJECT MANAGER (PM) <br />Mr. Steve Elliott: Acquaint field personnel with potential hazards and procedures to minimize <br />the negative impact of those hazards. Make available proper PPE and other safety equipment, <br />adequate time and budget and trained personnel to perform site work in a safe manner. <br />Arrange for preparation of an SSHP. <br /> <br />2.2 FIELD TEAM LEADER (FTL) <br />Mr. Kevin Weidman or Mr. Juan Stakes: Ensure that site personnel have read and will follow <br />the master copy of this document. Investigate and report to the PM each work-related illness or <br />injury, near misses, accidents and damage to physical property. The FTL will also serve as Site <br />Safety and Health Officer (see Section 2.4). <br /> <br />2.3 SAFETY & HEALTH MANAGER (SHM) <br />Mr. Alfonso Gamino: Be responsible for the development, implementation, oversight and <br />enforcement of the SSHP. Sign and date the SSHP prior to submittal as a reviewer. Be <br />available for emergency consultation. Provide consultation as needed to ensure the SSHP is <br />fully implemented. Coordinate any modifications to the SSHP with the PM, the SSHO and the <br />Contracting Officer (CO). Review SSHP plan amendments for specific site tasks. Review <br />accident reports and results of daily inspections. Serve as a member of the Contractor's quality <br />control staff. <br /> <br />2.4 SITE SAFETY & HEALTH OFFICER (SSHO) <br />Mr. Kevin Weidman or Mr. Juan Stakes: Implement and enforce the accepted SSHP while on <br />site. Be assigned to the site on a full-time basis for the duration of field activities. Conduct initial <br />site-specific training. The SSHO has authority to stop work if unacceptable safety or health <br />conditions exist and take necessary action to reestablish and maintain safe working conditions. <br />The SSHO will consult with and coordinate any modifications to the SSHP with the PM. The <br />SSHO will serve as a member of the Contractor's quality control staff on matters relating to <br />safety and health. The SSHO will conduct accident investigations and prepare accident reports. <br />The SSHO will review results of daily quality control inspections and document safety and <br />health findings in the daily reports.
