Site Safety and Health Plan <br />Stockton Field Maintenance Shop #24 <br />8020 Airport Way, Stockton, CA <br />15.0 LOGS, REPORTS AND RECORDKEEPING <br />Star personnel and all subcontractors shall maintain logs and reports sufficient to document the <br />implementation and execution of the personnel protection program and other relevant areas of <br />this SSHP. This documentation shall consist of medical surveillance files, training files, daily <br />logs and accident reports. These field forms should be filled out on a regular basis. Copies of <br />various forms and logs that will be used during the course of this project are included in <br />Appendix D. <br />15.1 MEDICAL SURVEILLANCE <br />Confidential medical surveillance files (Star employees) are maintained by the Star Corporate <br />Safety and Health Manager. These files document employee participation in the medical <br />surveillance program and fitness to work on hazardous sites. The Star Medical Director <br />maintains medical records. Access to these files is controlled by the Star Corporate Safety and <br />Health Manager. <br />15.2 TRAINING <br />Documentation of Star employee training is maintained in Star health and safety files under the <br />direction of the Star Corporate Safety and Health Manager. These files document employee <br />attendance, level of training and follow-up or refresher instruction. Any subcontractor will make <br />documentation of their employees' training available upon request. <br />15.3 WORK LOGS <br />Daily work histories shall be maintained by the Star SSHO. The daily work histories shall <br />contain (at a minimum): <br />Date; <br />Area(s) or site(s) worked; <br />List of employees by area; <br />Employee/visitor register; <br />PPE utilized by employees; <br />Training logs (site-specific/visitor); <br />Daily safety inspection logs; <br />Equipment maintenance logs; <br />Environmental and personal monitoring results; and <br />Description of special or unusual events or incidents, including all first-aid treatments <br />not otherwise reportable. <br />Any incident resulting in a work stoppage shall be fully documented in a report prepared by the <br />SSHO and submitted to the Star PM. <br />38 Star Resources Corp
