Stockton, CA <br />15.4 ACCIDENT REPORTING <br />In addition to descriptions in the daily log and work stoppage reports, an accident and/or <br />chemical exposure incident shall be investigated, analyzed and documented in an Accident <br />Investigation Report submitted to the Star PM. This process shall be applicable to both Star <br />employees and all subcontractors. These reports, prepared by the SSHO in consultation with <br />the Star PM, shall contain a full description and analysis of the incident, including exposure work <br />hours and a log of occupational injuries and illnesses (OSHA Form 300 or equivalent, as <br />prescribed by 29 CFR 1904). <br />Formal accident reports shall be prepared for any diagnosed illness or injuries that result in a <br />lost workday or fatality. The accident report shall identify all contributing causes and <br />recommend future hazard control measures to reduce the risk of recurrence. <br />Persons on site are responsible for reporting all injuries as soon as possible to the SSHO or the <br />Star PM. These injuries will be reported to the CA ARNG Contracting Officer and the site point <br />of contact. <br />15.5 JOB EXPOSURE REPORT <br />The SSHO shall complete a Job Exposure Report form and deliver it to the Star PM at the <br />termination of field activities at the site, in the event of assignment of a new SSHO at an <br />intermediate point during site activities, or at the end of discrete phases of prolonged field <br />activities. The purpose of Job Exposure Reports is to provide documentation of actual and <br />suspected job-related exposures for use by the Star Medical Director in making decisions on <br />appropriate periodic examination procedures. <br />Each Job Exposure Report shall be countersigned by the Star PM, who should ensure <br />appropriate compliance with this procedure. <br />15.6 DAILY TAILGATE/SAFETY MEETING CHECKLIST <br />The items to be discussed during tailgate/safety meetings are summarized on the tailgate <br />meeting form (Appendix B). All personnel present during the meetings are required to sign the <br />copy of the tailgate meeting form. <br />39 Star Resources Corp
