Environmental Health Department <br />AMENDED <br />4/26/2021 <br />Hazardous Materials Program Inspection Report <br />Date: <br />April 26, 2021 <br />Facility Address: <br />3900 HOLLY DR, TRACY <br />Facility Name: <br />TRACY WASTEWATER TX <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br />11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br />OBSERVATION: Wayne Acenas, Glenn Black, and Tasha Cheney, was last trained 04/17/2019. <br />REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel <br />receive initial and annual refresher training. All employees shall be trained in safety procedures in the event of a <br />release or threatened release of a hazardous material, including, but not limited to, familiarity with the following <br />plans and procedures: <br />(A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br />program agency. <br />(B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment. <br />(C) Evacuation plans and procedures, including immediate notice, for the business site. <br />This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br />years. <br />CORRECTIVE ACTION: Immediately provide employee training for appropriate personnel and submit a copy of the <br />training records to the EHD. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by 5/26/2021. <br />To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br />of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br />($152). To schedule an appointment, please call (209) 468-3420. <br />An inspection report was provided to the facility operator on the day of inspection, April 22, 2021. The report <br />was amended by the EHD. This amended report replaces the initial report. <br />Changes made; <br />Violation #9: Changed to corrected on site due to CERS submittal made on 4/22/2021. Removed corrective <br />action statement. <br />Violation #10: Changed observation to "During the inspection, Aloke Vaid stated that Sulfur Dioxide and Chlorine <br />had been removed since Aug 2019.
