Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: CO9ACHSRY Site Location: 21915 SEXTON RD <br />Activity Summary <br />Activity Date Recorded by <br />12 /30/2020 Received a call from William, tenant. He asked about the business card I left on GAITAN <br />the front door. I informed him that PGE spill behind his house was not cleaned up. <br />He stated that the property owner had contact him and notified him regarding the <br />matter. Let him know that clean up will resume today, he stated he was fine with <br />that. <br />12/30/2020 Received PSC report and additional photos from Celia Barajas, Environmental <br />Field Scientist, PGE. Reviewed photos and report. <br />Five-55 gallon drums of soil impacted with transformer oil were generated from <br />clean up. All five-55 gallon drums and soil sample were taken to PGE service <br />center located in 811 West J. St. Oakdale, CA. <br />GAITAN <br />12/30/2020 Generated prop 65. GAITAN <br />12/30/2020 3:18pm Met with Michael Levitt, Lead for Ancon Environmental. Stated that Ancon GAITAN <br />has been contracted to take over clean up and clean up is expected to take a total <br />of 3-4 hours. He stated they will continue based on visuals and will perform <br />notebook paper test to detect any oily sheen in the soil. Contacted Michelle Henry <br />'Lead Senior REHSI discussed complaint, based on discussion I will remain on <br />site until clean up is completed. Michael took three soil samples one from the <br />northern most point, one from the middle and one from the southernmost point of <br />the approximately 20 foot impact area. He placed each soil sample onto a sheet of <br />notebook paper, left it for 5 minutes. The middle sample resulted in a small oily <br />sheen. He excavated an additional inch of soil from the middle portion of the <br />impact area. He grabbed another soil sample and placed it on to the notebook <br />paper, no oily sheen was left behind after another five minutes. I grabbed a pair of <br />nitrile gloves and grabbed soil from the north, middle and southern portion of <br />impact area. No oily sheen was observed on nitrile gloves, the oily gloves were <br />disposed in one of the five 55-gallon containers. Clean up was determined to be <br />adequately cleaned. <br />An additional five -55gallon drums with soil impacted with transformer oil were <br />generated. Approximately 2-3 inches of additional soil were excavated. <br />In total ten 55-gallon drums with soil impacted with transformer oil were generated <br />from clean up. Approximately 4-6 inches of soil were excavated from clean up. <br />Michael stated that the Five-55 gallon drums were being taken to 4040 west lane <br />Stockton, Ca. PGE service station. He stated the soil was going to be going as <br />non-hazardous waste. I informed him that if the sample results for transformer oil <br />were to come back as hazardous, then he would be transporting waste soil without <br />proper labeling. He understood. <br />Contacted Celia Barajas, Env. Field Scientist and updated her on clean up. She <br />stated she would have sample results and disposal receipts to me by tomorrow. <br />Left site 4:30 pm <br />12/31/2020 Prop 65 signed and reviewed by Stacy Rivera 'Lead Senior REHSI. Submitted to GAITAN <br />prop 65 in box. <br />01/04/2021 Sent email requested PCB results from Celia Barajas, Env. Field Scientist PGE. GAITAN <br />Received and reviewed an email sent by Celia Barajas attached with PCB results. <br />Soil sample taken on 12-29-21 from Orchard located on 21519 Sexton Rd. <br />Yielded non-detect for PCB. <br />01/05/2021 Generated photo logs from photos taken by myself on 12-30-2020 and photos GAITAN <br />given to me by Hydrochem PSC. <br />5105 rpt <br />Page 3 of 4
