Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: CO9ACHSRY Site Location: 21915 SEXTON RD <br />Activity Summary <br />Activity Date <br />12/30/2020 <br />Recorded by <br />Generated notice to abate and sent copy to Celia Barajas, Environmental Field GAITAN <br />Scientist, PGE. <br />Arrived at 21915 Sexton Rd. Could not locate spill area, contacted Celia and <br />asked for directions on where the spill area was located. She stated that she does <br />not know, she said she would contact PSC foreman, Anthony and get back to me. <br />Received a call from Celia she could not get a hold of Anthony, she provided an <br />additional address to see if I can find spill area. I went to the following addresses: <br />-2373 Sexton Rd. Escalon, CA 95320 <br />-22339 E. River Rd. Escalon, CA 95320 <br />I observed no spill area at both locations. Celia sent a message and suggested I <br />contact property owner to locate spill. <br />I contacted Glen Vann Ruler, property owner and left a voicemail. Received call <br />from Celia, she stated that the spill is located on 21915 Sexton Rd. behind the <br />property. She stated that she was on her way, I responded that I would wait for <br />her. Went to 21915 Sexton Rd. knocked on door of house located on property, left <br />my business card after no answer. Began writing narrative. Celia arrived and <br />showed me spill area. <br />Spill area is located approximately 800 feet behind the house. We took the dirt <br />road on the side of the house which lead to the replaced pole mounted <br />transformer. I observed an approximately 10 by 20 foot spill area, I noticed some <br />heavy staining on some spots in the impact area. Most notably the border of the <br />northern most impacted area appeared darker than the rest of the impact area. <br />Discussed site with Celia, she stated that the darker spots were due to soil being <br />exposed underneath, after digging and potential watering of the orchard. I stated <br />that the clean up did not appear done. She contacted Anthony, foreman for PSC, <br />and asked how much soil did they shovel out. He stated that they dug <br />approximately 2 to 3 inches depending on the area and generated approximately <br />five 55-gallon containers with soil impacted with transformer oil. I asked Anthony <br />how he determined that area was cleaned; he stated they kept digging until there <br />was no odor. I grabbed a pair of nitrile gloves and grabbed soil from the impact <br />area from the northern most point into the orchard. I noted a shimmering residue <br />left behind on the gloves. Celia stated that maybe it was from water, I informed her <br />that water does not typically leave a shimmer on the gloves. I grabbed background <br />soil sample outside of the impact area with a clean pair of nitrile gloves; I <br />shoveled with my hand approximately 1 inch down. I held the soil in my hand and <br />showed Celia that the soil that was not impacted by transformer oil did not leave a <br />shimmer behind on the glove. I did this two more times from different soil areas. <br />Celia performed her own test and placed soil from the impact area onto a piece of <br />notebook paper. After a couple of minutes, she dumped the soil back into the <br />impacted area and noted that an oily residue was left behind on the notebook <br />paper. She agreed that additional cleaning was required. <br />I contacted Michelle Henry 'Lead Senior RENS' and updated her on the complaint. <br />Celia informed me that she will contact PSC and notify them that additional <br />cleaning is required. Went back to EHD and labeled and deposited nitrile gloves <br />contaminated with transformer oil into the hazardous waste storage closet. <br />Received call from Glen Van Ruler, property owner, and discussed clean up, I <br />notified him that I was on site with PGE and clean up is not complete. I stated <br />additional clean up would have to be made. He stated he was okay with additional <br />clean up being performed. <br />12/30/2020 Sent message to Celia Barajas, Env. Field Scientist, PGE. Requested photos that GAITAN <br />PSC took when clean up was finished and ETA for PSC to perform additional <br />clean up. She stated that she will send me the photos and PSC will be on site at <br />230pm. Notified her that I will be there. <br />51 05. rpt <br />Page 2 of 4
