This violation <br />was cited July 31, 2018 and was corrected July 24, 2019. <br />3.3 The Respondent violated HSC 25508(a)(1), failed to complete and/or <br />submit the business activities or owner/operator identification page. The <br />business activities and owner/operator identification forms had not been <br />submitted. This information must be current and submitted as part of the <br />business plan. This violation was cited July 31, 2018 and was corrected July <br />24, 2019. <br />3.4 The Respondent violated HSC 25506(a), failed to complete and/or <br />submit hazardous materials inventory <br />for all reportable materials on site. <br />48 * 8 gallon propane cylinders, a 350 gallon diesel tank, a 200 gallon <br />gasoline tank, a 55 gallon drum of waste oil and greater than 200 cubic feet <br />of compressed oxygen were found an site that had not been reported. Any <br />material that meets or exceeds the reportable quantity shall be reported in <br />the facility's business plan.
