Take all necessary precautions to ensure testing and submittal of <br /> test reports are <br /> performed in a timely manner. <br /> UPS respectfully disputes this finding and request it be rescinded as a copy of the email was sent <br /> to your office on 8/13/20 See Attachment C which contained the certification report conducted on <br /> 7/15/20, sent by our testing contractor RL Stevens' technician Dave Pereira. Report submittal via <br /> email is and has always been our primary means of submittal of the test report within 30 days of <br /> testing. The inclusion of the report in our annual HMBP submittal through CERS that was done <br /> on 12/30/20 is not our means of fulfilling that requirement. <br /> Item#202 <br /> CCR 2712(1) Failure to retain a paper or electronic copy of the permit to operate at the <br /> facility. <br /> OBSERVATION:A copy of the UST operating permit was not found on site. <br /> REGULATION GUIDANCE:A copy of the UST operating permit and all attachments and <br /> conditions shall be <br /> retained at the facility. <br /> CORRECTIVE ACTION. Immediately obtain a copy of the permit and retain it on site. <br /> UPS respectfully disputes this finding and request it be rescinded as we paid the annual CUPA <br /> permit renewal earlier this year but never received the paper permit that was mailed to our <br /> Oakland facility. Because we never received a copy we contacted Paul Nso from the county <br /> office, and still yet to receive a valid copy from your office. Please see Attachment D as a copy of <br /> the email communications. We requested copies from your office and did not receive any follow <br /> up. <br /> At this time, we would like to again make an endeavor to ask your office for a copy of the 2021 <br /> permit either by mail or email, or both. <br />
