ATTACHMENT A <br />Requirements for Monitoring Well Abandonment Reports <br />After abandonment of any groundwater monitoring well, the Discharger shall submit a report <br />containing, at a minimum, the information listed in below. All reports must be prepared under the <br />direction of, and signed by, a registered geologist and civil engineer licensed by the State of California. <br />Minimum Contents for a Monitoring Well Abandonment Report <br />A. General Information <br />Purpose of the well abandonment project. <br />Brief description of local geologic and hydrogeologic conditions. <br />Identification and number of monitoring wells abandoned. <br />Topographic map showing facility location, roads, and surface water bodies. <br />Scaled site map showing the location of abandoned wells <br />B. Original Well Construction Details <br />Well numbers and dates installed. <br />Diagram with well construction details. <br />Copy of original boring logs. <br />C. Water Well Standards, Section 23 <br />The Department of Water Resources Water Well Standards, Section 23 is available at <br />http://www.water.ca.qov/oroundwater/well info and other/california well standards/wws/wws <br />combined sec23.html <br />Results of preliminary work to determine the wells condition, construction details, and <br />whether there are obstructions that will interfere with the process of filling and sealing, <br />as required in Section 23 of the Water Well Standards. <br />Discussion of filling and sealing conditions, as required in Section 23 of the Water Well <br />Standards. <br />A discussion of the abandonment and materials, including the distance between the top <br />of the cement grout and the subgrade of the disposal module. <br />Diagram with placement, distance between top of cement grout and bottom of subgrade <br />elevation of the disposal module, depths of placed materials, and types of material <br />placed for abandonment. <br />D. County Requirements <br />Copy of the application to the county and the permit. <br />Signed copy of County's observations during the abandonment. <br />E. Table with Well Survey Data <br />Coordinate system, epochs, and northing, easting, and elevations <br />Date survey performed. <br />Name of registered land surveyor. <br />KARL E.
