3: Hazard Analysis Health and Safety Plan <br />Prior to use, inspect cords for exposed or bare wires, worn or frayed cords, and incorrect <br />splices. Splices are permitted, but there must be insulation equal to the cable, including <br />flexibility. <br />Cables and extension cords in passageways, steps or any area where there may be foot traffic <br />should be secured so as to not create a tripping hazard. Overhead cables and extension cords <br />shall be rigged to a height greater than 6 feet. <br />Shield extension cords that must run across driveways or areas where vehicle traffic is present. <br />Do not run cords across doorways or windows where they can be frayed or cut by a closed <br />door or window. <br />Do not run wires through wet or puddled areas. <br />Flexible cord sets that are used on construction sites or in damp locations shall be of hard <br />usage or extra hard usage type. <br />Observation of energized machinery will take place from a safe distance. Only qualified personnel <br />will remove guards, hatch covers, or other security devices if necessary. Equipment lockout <br />procedures and an appropriate facility work permit requirements will be followed. Lockout/tagout <br />procedures will be conducted before activities begin on or near energized or mechanical equipment <br />that may pose a hazard to site personnel. Workers conducting the operation will positively isolate <br />the piece of equipment, lock/tag the energy source, and verify effectiveness of the isolation. Only <br />employees who perform the lockout/tagout procedure may remove their own tags/locks. <br />Employees shall complete lockout/tagout training before initiating this procedure. <br />Only qualified personnel will remove covers of electrical equipment to expose energized electrical <br />parts. Entering electrical rooms/vaults or areas with live exposed electrical part by BC employees <br />shall be permitted only when accompanied by a qualified personnel after notification and approval <br />of the appropriate facility personnel. <br />3.4.12 Fire/Explosion <br />Site workers should have an increased awareness concerning fire and explosion hazards whenever <br />working with or near flammable materials, especially when performing any activity that may generate <br />sparks, flame, or other source of ignition. Intrinsically safe equipment is required when working in <br />or near environments with the potential for an explosive or flammable atmosphere. The SSO will <br />verify facility requirements for a "hot work" permit before activities that may serve as a source of <br />ignition are conducted. <br />Flammable materials will be kept away from sources of ignition. In the event of fire, work will <br />cease, the area will be evacuated, and the local fire response team will be notified immediately. Only <br />trained, experienced fire fighters should attempt to extinguish substantial fires at the Site. Site <br />personnel should not attempt to fight fires, unless properly trained and equipped to do so. A fully <br />charged ABC dry chemical fire extinguisher will be readily available for use during all scheduled <br />activities at the Site. <br />BROWN Arco CALD WELL <br />3-10
