Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: C00052275 Site Location: 0 ARCH RD <br />Activity Summary <br />Activity Date Recorded by <br />07/15/2020 5:00 pm: Continuation from day time ER. Arrived onsite (5:09 pm), the the City of MANZO <br />Stockton Storm Water Division representatives (Bryan Toledo; (209) 937-8719 <br />and (209) 993-3273, and Road Maintenance was onsite at the time of arrival, as <br />well as Stockton Fire and Police Departments (John Titus). Two large vehicles <br />belonging to JB Hunt had burned down, the Fire Department was in the process of <br />finishing up putting out some of the smoke. Observed a large pile of burnt oily <br />waste rubble and a large pool of oily water entering nearby storm drains. There <br />was a substantial fire damage to the roadway and the surrounding poles and <br />traffic lights. The Fire Department utilized a large quantity of water to put out the <br />fire. Met with Mike Austin, a senior adjuster/ investigator from David More and <br />Associates, who was representing JB Hunt; discussed the clean up requirements <br />with Mr. Austin. Spoke several times with JB Hunt claims department via phone <br />(479-419-3000), a claim was generated for this incident # 20359576. Salazar and <br />Dave's Tow companies removed the burnt truck remains and trailers offsite. One <br />trailer carried igloo coolers, and one carried baby cribs. No hazardous materials <br />were reported inside the cargo containers. A large burnt oily rubble pile was left <br />after the trucks and trailers were towed away, this large pile was blocking the <br />intersection and a turn lane at the Logistics and Arch intersection, the road had to <br />remain closed in both directions pending the clean up. Participated is several <br />discussions with JB Hunt claims and Mr. Austin onsite concerning the clean up. It <br />toos some time for a clean up company to be retained by JB Hunt. Prepared a <br />notice to abate and emailed to maustin@davidmorse.com with cc to Bryan Toledo <br /><Bryan.Toledo@stocktonca.gov >; and Lydia Baker (EHD) lbaker@sjgov.org. I <br />was escorted by Bryan Toledo (the City of Stockton Storm Water Division) to <br />inspect the nearby lift station and a catch basin at Newcastle Road where all the <br />contaminated stornnwater drained to. Observed sheen on standing water and <br />dieseVburn odor inside the lift station, some standing water was also present in the <br />concrete gully of the retention basin, this area was not accessible to ascertain the <br />water conditions. Since the clean up crew was retained and scheduled to arrive <br />today, but was not arriving immediately, left the site.
