Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: C00052275 Site Location: 0 ARCH RD <br />Activity Summary <br />Activity Date Recorded by <br />07/15/2020 8:32 pm, called the number provided by the JB Hunt Claims Department MANZO <br />(812-629-7300), spoke with Premium Environmental dispatch,the clean up broker, <br />and obtained the information for the clean up company (Encon) that was <br />responding onsite. Nolan Ryan is the contact 916-701-3376. Confirmed that the <br />clean up crew will be onsite at approximately 20 minutes. The plan is for the <br />Encon to remove the burn waste from the right of way and secure it, so the road <br />could be reopened. No storm drain clean up is scheduled for today, due to <br />logistical issues. Sent a text to Bryan Toledo at 415-299-9923, to update the ETA <br />for clean up crew arrival. 8:45 pm left to go to the site. 9:17 pm, arrived onsite, <br />several individuals from the Encon crew were onsite, but they were waiting for the <br />rest of the equipment to arrive. No clean up had started after the rest of the crew <br />arrived at around 10 pm. Called JB Hunt claims department and confirmed claim <br />and billing information. Encon clean up crew didn't bring a roll off bin to secure the <br />waste, so all the waste was removed from the road and placed on a tarp on the <br />nearby sidewalk area. Although I indicated to the City of Stockton Representatives <br />(and to the clean up company) that absorbent application to the stained roadway is <br />sufficient, on City of Stockton's request, the entire intersection area and gutters <br />leading to the three nearby storm drains were pressure washed. All rinsate was <br />recovered with a vacuum truck. Two storm drains were impacted. <br />Called Chris Buckley with Stockton Fire Department at 209-612-4373 and left a <br />message. <br />Called Stockton Fire Department at 209-937-8801 and they transferred me to the <br />Chief on scene. He could not provide responsible party information at the time. <br />He said both drivers are still on site. Two storm drains were impacted and the City <br />of Stockton is on site helping with the clean-up. <br />07/16/2020 Received an updated spill report for the incident from Kim Blackwell. ALANIZ <br />Discussed the incident with Elena Manzo, Lead Senior.
